HOW TO APPLY • Apply for EITHER the two AAFP funded scholarship opportunities per the attached instructions. • The Family Medicine Leads Scholarship National Conference Scholarship • The Family Medicine Leads Emerging Leader Institute. • Send us a copy of your application(s). • Send us a signed copy of the attached consent for the AAFP to release to us your rating in the application review process. *Email all documents to Gerry.stover@kafp.org

Family Lead National Conference Scholarship • Purpose: To encourage the role of family medicine • Program Period: July 28, 2016 to July 30, 2016 • Funding: $600 •  See what family medicine is all about and discover the scope of practice offered in the specialty • Workshops • main stage sessions • musculoskeletal clinics. • LGBT health screening • Maternal care and childbirth • Topics include: • Financial responsibilities • From residency to faculty • Health disparities

  10. What YOU Need to Do Eligibility Requirements Submission Guidelines The online application does not allow you to save and return to resume the application at a later time. Before starting your application, have all materials and information listed below ready and in the required format. Contact information Year in training (during the 2016-17 academic year) Essay attachment (include name at top of essay; preferably save as .pdf). Prepare a 500-word, one-page typed essay on the following topics: Explain why you are interested in family medicine and why you want to attend National Conference. E-Signature • You must be a member of the American Academy of Family Physicians (AAFP) to apply. Join now. • You may apply for one scholarship annually. • If you have previously received a Family Medicine Leads Scholarship, you are not eligible to apply for the same scholarship again. • To apply for a student scholarship you must be a medical student during the 2016-17 academic year. You are not eligible to apply for a student scholarship if you will graduate before July 1, 2016.  • You must be able to attend the entire National Conference. • You must agree to the Family Medicine Leads Scholarship Terms of Agreement (1 page PDF attached). Submit this with your application.

Click the first option that says FIRST TIME STUDENT ATTENDEE. ***You MUST complete the application in SAME SETTING. It WILL NOT let you save and resume at another time.*** The required online application must be completed and submitted to the AAFP by May 1, 2016.  Winners will be notified by May 31st. APPLY ONLINE

The Emerging Leader Institute • Purpose: To identify future leaders of family medicine • Program Period: July 28, 2016 to July 31, 2017 • Funding: $1,000 provided to attend BOTH the National Conference AND the Emerging Leader; Up to $3,000 possible at the end of the program period • Attend post-conference training event with three possible tracks: • Policy & Public Health Leadership • Personal, & Practice Leadership • Philanthropic & Mission-Driven Leadership. • Implement a year long research project with an assigned mentor

Don’t Reinvent the Wheel! • For your application, you must develop a potential research project idea for each of the three tracks: • Policy & Public Health Leadership • Personal, & Practice Leadership • Philanthropic & Mission-Driven Leadership. **If you are already working on a project for a class, tweak it to fit each of the three tracks instead of coming up with a completely different project idea for each track.

Project Tracks PHILANTHROPIC & MISSION-DRIVEN LEADERSHIP • Maximizing your impact as a physician leader as you increase your income, gain professional respect, and choose among many organizational opportunities for involvement. • Strategies for becoming a "change agent" in your community • Your personal mission statement • Leadership; competencies and skills "gap" • Non-profits and your philanthropic goals • Understanding non-profits POLICY & HEALTH LEADERSHIP • Key strategies to move our specialty into a position to optimize the future health of America— both broadly and within local communities. • Effective leadership skills in policy and public health • Improve population health • Current AAFP policy and public health focus • Roles and strategies to engage in policy interventions • Effective community health engagement PERSONAL & PRACTICE LEADERSHIP • Remaining effective and focused as you transition to greater levels of responsibility, autonomy, and expectations. • Personal leadership "toolbox" • Key trends in practice management and leadership opportunities • Strategies for burn-out avoidance • Work/life balance • Integrate high level healthcare thinking into your work

What YOU Need to Do Eligibility Requirements Submissions Guidelines Three documents are required in order to complete the application process: Family Medicine Leads Emerging Leader InstituteApplication(Must be submitted electronically) Curriculum Vitae (CV) (May be submitted in Word or as a PDF) Letter of Recommendation • Must be a member of the American Academy of Family Physicians  • Must be in second, third, or fourth year of medical school when program begins • Must receive pre-approval from medical school director for participation in this program
