No cheques ​ Please bring the following with you: A completed form  A patient summary from your GP A valid passport for ID purposes Last Medical Certificate- if applicable Glasses/ contact lenses if worn with an up to date spectacle prescription  A Chaperone if you wish A means to pay for the assessment on the day www.flyingmedicine.uk
