Good Social Care is the cornerstone of the an effective health and social care service Improvement and implementation of plans • The five BCF fast track areas prove this: • 5,466 fewer unplanned hospital stays • 8229 fewer unnecessary days spent in hospital by helping people get home without delay • 642 older people remaining at home at least 3 months after discharge from hospital • 98 people supported to live independently rather than move to a care home #AskBCF

This is about long-term sustainability of the NHS Improvement and implementation of plans • Evidence from the 5 fast tracks shows: • £279.6m pooled in the Better Care Fund across the 5 sites • Financial savings of c.£33m from the 5 sites (Greenwich: £3.9m, Nottinghamshire £7.87m, Reading £1.76m, Sunderland £12.38m, Wiltshire £7.47m) • £154m invested in social care through the BCF (Greenwich: £9.9m, Nottinghamshire £22.7m, Reading £4.3m, Sunderland £94.9m, Wiltshire £22.3m) 64% of trusts are concerned about the financial and operational sustainability of their Trust in the next 5 years - FTN, 2014 “Our latest quarterly report paints a picture of a service under huge pressure, with cracks beginning to appear in NHS performance” John Appleby, Chief Economist Kings Fund, July 2014 #AskBCF
