Empowering Older Adults in Sunderland: Promoting Wellbeing and Social Inclusion

Our initiative aims to maximize the support for older adults (65+) in Sunderland by enabling participation in social activities and reducing reliance on institutional care. We collaborate with external agencies to enhance our outreach and promote healthy, active lifestyles. Eligibility criteria include a willingness to work towards personal goals and a support need for stability, health access, and social inclusion. Since October 2012, we’ve identified and contacted over 2,600 potential customers, providing vital services to enhance wellbeing, safety, and community engagement.

Aims/Objectives • Maximise number of people supported to participate in social activities Reduce the number of people entering institutionalised care • Work alongside external agencies - Joint Partnership working Increase the number of people supported across the city Promote Healthy & Active Lifestyles

Eligibility Criteria Aged 65 and over Must live in the City of Sunderland – All tenure eligible Must have a support need to: Develop a stable lifestyle Access other appropriate services Access health services Live safely & securely Become socially included with friends/family and/or their local community Be willing to work with staff on an outcome based plan that defines future aspirations

Gentoo Customer Findings Since 1st Oct 2012 We have: Actively identified 3875 potential customers Contacted 2662 of those potential customers Recorded a 28% acceptance rate of a wellbeing service Examples Of Outcome Feedback From Customers Who Received The Service: Felt safer – 23 Improved ability to manage unassisted – 22 Improved independence - 8 Prevented future falls – 6 Better off financially – 5 Increased social contact - 2
