This workshop focuses on a new crisis intervention home support model for individuals with learning disabilities. It aims to prevent hospital admissions, ensure care in safe home environments, and expedite discharges. The model offers choices for clients, such as admitting to the hospital or referrals to specialists. Common reasons for admissions include severe behavioral issues, medication stabilization, and breakdowns in home circumstances. Funded by DHSSPS, this pilot project will be evaluated in 6 months to assess its effectiveness in providing flexible, responsive care tailored to individual needs.
