  4. Palpitation – Mechanism • LVH → Recognization of Heart beat near thoracic wall • Multiple cause → Increase Catecholamine Exercise, Fever, Hyperthyroidism, Pheochromocytoma, Anemia, Drug, Pregnancy, Agitation

  5. Palpitation – DDx.

  6. Palpitation – Hx. taking • Taping Table – Arrhythmia ! • 섬세한 리듬감각이 필요 • Situation, RF Exposure, Drug, Family Hx. • Associated Symptoms

  7. Case relevance • 두 번째 EKG 상 AF 관찰됨 • 심방수축소실 → 심실 filling 저하 → CO 감소 → 현기증, 피로, 전신 쇠약감, 교감신경 항진 • But 특별한 이상 소견 관찰 X : 평소 운동으로 인한 보상기전? • 교감신경 항진으로 인한 Palpitation → 불안 초래할 수 있음

  9. Innocent murmur – Characteristics • Short (early to mid-systolic, except for the venous hum) • Low to medium pitch • Possibly a musical component • Normal physiological splitting of S2 • Commonly found in children & early teen year

Innocent murmur Adult : carotid artery stenosis Children : Innocent Carotid bruit 목에서 경정맥을 따라가며 들리는 잡음 정맥을 누르면 사라지거나 목 자세를 바꾸어도 변하는 것이 특징 Venous hum 청진 부위 : 2nd Lt. intercostal space 짧은 수축 중기 음 운동시 murmur 증가 Pulmonary flow murmur Vibratory systolic murmur (Still murmur) 3~7세 에서 가장 흔함(40~80%) 수축기Low-pitch murmur Apex에서 잘 들림 누우면 잘 들리고 서면 줄어들거나 사라짐

  14. Maze Procedure - General • To treat patients with atrial fibrillation • by reducing atrial tissue mass • maintaining sinus rhythm with AV nodal conduction.

  17. Maze Procedure - Indication • 동반 외과적 적응증이 있는 6개월 이상의 병력을 가진 심방세동 • 내과치료(drug, radiofrequency ablation)에 반응하지 않는 심방조동 • 특히 발작성(paroxismal)인 예에서 환자가 적응하지 못할 때 • 부정맥약제에 심한 부작용 • 혈전색전증의 병력 • 심전도상 V1 lead의 f wave가 1mm 이상 • 좌심방의 크기가 70mm 이하 • 단순흉부촬영상 심장흉곽비가 70 % 이하

  18. Maze Procedure – ETC • Cox -두차례의술식변형 • Outcome 향상, 수술 방법 쉬워짐 • 복잡, 허혈시간의 연장 • Maze술식의 단순화(?) - 변형술식 시도 • Mortality, morbidity 영향 Not sure!! • Defibrillation 효과 미흡 • 참고 • 심장 재수술 시 MAZE 술식 병행 시행에 따른 위험도 및 효과분석, 부천세종병원 흉부외과

  19. Case relevance • Maze procedure의 Ix. 중 • 동반 외과적 적응증이 있는 • 6개월 이상의 병력을 가진 • 심방세동 ASD, severe TR, mild MR 올해 초 부터심계항진 증상 (AF로 인한 걸로 추측됨)
