  12. MINIMUM REQUIREMENTS FOR ACCREDITATION Practitioner in Occupational Safety and Health (OSH Practitioner) a.1. Must have completed the Bureau prescribed 40-hour BOSH Training Course from DOLE accredited organizations. a.2. Must have relevant experience in occupational safety and health. (3) yrsexperience is required if applicant is duly licensed, (4) yrsexperience, if graduate of any 4 or 5 year college course without license (10) yrsexperience if college undergraduate. Relevant experience shall mean actual work experience on OSH or a combination of actual work experience and attendance or

  13. What are the documentary requirement ? (for evaluation/validation) • Duly accomplished Application Form • (DOLE-BWC-AF-PCN-A1). • 2. Updated Original Certificate of • employment • 3. Original copy of actual functions/job d escriptionduly signed by the employer or authorized official of the company. • Photo copy of previous employment • certificate

  14. What are the documentary requirement ? (for evaluation/validation) • 5. Photo copy of Certificate of Completion on the Bureau prescribed BOSH training course issued by DOLE Accredited STOs (minimum of 40 hours) • 6. Photo copy of Certificate of Attendance/ Participation to other OSH related trainings/ seminars from DOLE accredited / recognized organizations/institutions.

  15. What are the documentary requirement ? (for evaluation/validation) • 7. Photo copy of college diploma, transcript of records and/or PRC license, if any • 8. Proof of applicant’s achievements/ accomplishments/participation on OSH • ( i.e. awards/recognitions, OSH reports) • Note: All pages of documents submitted must be signed by the applicant and original of all documents submitted must be presented to the authorized evaluator during applicant’s interview.
