  2. Materials Needed • Blank voter registration forms • Black or blue pens • Clipboards

  3. EXAMPLE OF A COMPLETED VOTER REGISTRATION FORM • This form can also be used to update: • New home address • Political affiliation • Name change

  4. STEP 1:Make sure the applicant is qualified to vote: IN ORDER TO VOTE, THE APPLICANT MUST BE: • Citizen of the United States • At least 18 years old (or will be by next election day – November 6, 2012) • Not in prison or on parole for a felony

  5. STEP 2:Fill out personal information Mandatory Information: • Legal name • Home address • Date of birth and place of birth • California driver’s license number (or last four digits of social security number) • Political party registration or check no Optional Information: • If the applicant would like to receive ballots by postal mail, initial here • Mailing address (if different from home address • Email/phone number

  6. STEP 3 (OPTIONAL):If the applicant is re-registering • The applicant may already be registered to vote but need to change his/her: • Home address due to a move • Name • Political affiliation

  7. STEP 4:Citizenship/Age & Signature • Make sure the applicant checks yes to confirm that he/she is a US Citizen and over 18 years old • Make sure the applicant signs and dates the form

  8. STEP 5:If you helped fill outor deliver the form… • You must complete and sign this section with YOUR information if you help fill outordeliver this form for the applicant • If you’ve only helped them with the form but didn’t fill it out for the applicant and the applicant delivers it, you do not have to sign

  9. FINISHED! • Make sure the form is complete and signed • The applicant can either mail in the form or you can mail it in for him/her • If you mail it the form in for the applicant, you must do so within 3 DAYS. The applicant takes the receipt portion of the form. • FYI: You can check voter registration status at: http://www.lavote.net/
