  3. Benefits Overview • Jack Link’s provides a comprehensive benefits package to help protect your health, wealth, and your well-being. • You have the opportunity to customize a benefits package that meets your personal needs.

  4. Benefit Eligibility Overview • Full-Time employees are eligible the first of the month following 60 days of employment (except the 401(k) plan) • 401 (k) eligibility is first day of the new quarter (January 1, April 1, July 1, October 1) after 6 months of employment • 30-day grace period from eligibility date to enroll in benefits

Benefit Elections • 2013 plan year will run February 1- December 31 • In 2014 we will change our plan year to January 1 – December 31 • You may only make changes to your benefits during the year if you have a qualifying life event including: • your marriage, divorce or legal separation • death of your spouse or covered child • birth or adoption of an eligible child • a change in your or your spouse’s work status that affects benefits • a change in your child’s eligibility for benefits • a change in residence or work site that affects your eligibility • receiving Qualified Medical Child Support Order (QMCSO) • All paperwork must be submitted to HR no later than 31 days from the event • Benefit coverage will end on the last day of the month in which you terminate employment, loss of eligibility or failure to make contributions.

Viverae Engage Program • Jack Link’s has partnered with Viverae, a best-in-class wellness program provider • Health Tools will be available such as: • Personal Health Assessments • Nutrition and Physical Activity Trackers • Educational Resources • Creates Risk Factor Reduction and Behavior Change

Health & Wellness • Viverae Engage Program Incentive • $50 per month ($600 per year) • Incentive will be paid in a lump sum in January of the year following successful completion • Successful completion in 2013 = Incentive paid in January 2014 • Spouses are eligible to receive incentive if they complete the requirements and the employee has also successfully completed the program • Program Requirements • Enrollment in one of the company medical plans by March 1st • Complete Member Health Assessment (MHA) and Biometric Screening by April 30th • Complete Preventive Care Compliance • Earn an additional 50 points through program activities by December 31 • Employees eligible to enroll in a company medical plan between April 1- October 1 can still participate. The MHA and Biometric Screening must be completed within 60 days of eligibility date

Medical Plan Resources • Mayo Clinic 24 Hour Nurse line– Ask Mayo Clinic • Anytime, Anywhere- you have access to a registered nurse to help you decide when you need to see a doctor, discuss home treatment options, and answer questions about a medicine you’re taking. • Visit www.jacklinksnurseline.com Access Code: ASKJACK • Call 1-800-766-0014 • The Ask Mayo Clinic number will be on the back of your Medical ID Cards • Healthy Start Prenatal Support Program- 1-866-489-6948 • This program gives advice and personal attention to individuals during their pregnancy • $50 prepaid debit card upon completion of the program • Online resources available at www.myhealthystart.org • Fitness Discount Program– Save $20 on your monthly health club membership when you work out 12 or more days each month.

Flexible Spending Accounts (FSAs) • Jack Link’s offers two flexible spending account opportunities: • Health Care FSA - maximum of $2,500 • Dependent Care FSA - maximum of $5,000 • Expenses must be incurred on or before March 15, 2014 • Claims must be submitted by March 31, 2014 for reimbursement • Debit Cards • Participants will automatically be sent a debit card at no charge • Use your debit card to pay for medical expenses • The FSA accounts are administered by SelectAccount • You must complete a Select Account enrollment each year to receive this benefit • Visit www.selectaccount.com for more information IRS Guidelines:www.irs.gov “Search Forms & Publications” 502 – Health Care FSA 503 – Dependent Care FSA

  16. Dental Coverage

  17. Delta Dental Plan *Dentists who have signed a participating network agreement with Delta Dental have agreed to accept the maximum allowable fee as payment in full. Non-participating dentists are not obligated to limit the amount they charge; you are responsible for paying any difference.

  21. Life & Disability Coverage

  22. Life Insurance • Jack Link’s pays the entire cost of basic life insurance for full-time employees • The benefit is $25,000 • Contact Human Resources to designate or update your beneficiary

Voluntary Life Insurance • You may purchase additional life insurance coverage for yourself and/or dependents • Employees can purchase Voluntary Life insurance for yourself and Dependent Life for your spouse and dependent children, in the following coverage amounts: • Employee – In increments of $10,000 to a maximum of $500,000 ($200,000 Guarantee Issue) • Spouse- In increments of $5,000 to a maximum of $100,000 ($50,000 Guarantee Issue) • Child(ren) • $10,000 or $20,000 • You pay the full cost through bi-weekly payroll deductions • See page 15 of your benefit guide for a rate schedule

Disability Insurance • STD coverage • STD waiting period: 1st day injury/8th day sickness • Benefits will be paid for a maximum of 13 weeks • Your benefit is 60% of your weekly salary to a maximum of $500/week • Jack Link’s pays the entire cost of employees Short Term Disability (STD) • You pay the entire cost of Long Term Disability(LTD) • Disability insurance is really income replacement protection. • Disabilities affect 15% of all Americans (41 million people) • LTD coverage • LTD waiting period: 90 days • Benefits will be paid for a maximum of 5 years • Your benefit is 60% of your monthly salary to a maximum of $5,000/month

  28. 401(k) Retirement Plan • Jack Link’s offers a 401(k) plan through Fidelity • You may contribute a % of pay or flat dollar amount each pay period • Deferral amount can be changed at any time • Jack Link’s will match $.25 for every dollar you contribute up to a maximum of 4% of your compensation • Roth Anyone? • Jack Link’s Beef Jerky retirement plan offers both traditional pre-tax deferrals and Roth after-tax deferrals
