  2. Open Enrollment • Open enrollment April 18 – May 20 • All changes effective September 1, 2011 • No premium changes except for TRS-ActiveCare • You have to re-enroll for Flexible Spending • Critical change for declining coverage for TRS-ActiveCare • Further information on slides four and five

Changes for TRS-ActiveCare2011-2012 • Extension of coverage to dependent children until age 26, whether or not they live with their parents, are claimed as a dependent on a parent’s tax return or are married • Enrollees may make plan changes (AC1, 2 or 3) during the plan year due to a special enrollment event • Individuals who voluntarily drop coverage during the plan year may now re-enroll during the plan year due to a special enrollment event

Changes for TRS-ActiveCare2011-2012* Critical Change * • Certificate of Credible Coverage • No longer accepted in lieu of a declination form • DECLINATION • Declining coverage for yourself • If you are declining enrollment with TRS-ActiveCare, you must complete a Declination form each year • Declining for dependents • If you, the employee, are electing coverage with TRS-ActiveCare, but declining coverage for any dependents (including spouse), you must complete a Declination form listing your dependents and reason for declination • Declination Form

Critical Change Cont… • If you or your spouse lose your coverage due to loss of spouse job or death of spouse and you have not completed the Declination form, you WILL NOT be allowed to enroll with TRS-ActiveCare until the next plan year. • You will need the Certificate of Credible Coverage for pre-existing exclusions.

Other Open Enrollment Information • TRS-ActiveCare Premiums 2011-2012 • Enrollment Representative Schedule by Campus • LIFE INSURANCE & ACCIDENTAL DEATH & DISMEMBERMENT (MetLife) No premium increase • You may elect new coverage or increase existing coverage. However, you must complete a Statement of Health form and return it to Babs Holly. • CANCER (American Public Life) No Premium increase • During this open enrollment, anyone may elect cancer coverage. Even if you currently have cancer or have had it in the past, you are eligible to enroll and start receiving benefits after a one year waiting period. • DISABILITY (Unum) No premium increase • You may cancel, enroll, increase or decrease your disability coverage. There will be a pre-existing condition waiting period of one year for changes and new enrollments. • DENTAL (Guardian) – No premium increase • You may enroll, cancel or change plans • GROUP LONG-TERM CARE (Genworth) • Contact Genworth 866.659.1970 or www.genworth.com/trsactivemember for additional information

Open Enrollment Information Cont. • Cancelling coverage: • If you currently have TRS-ActiveCare and choose to cancel the coverage for 2011-2012, you must waive the coverage in the Employee Benefits HUB and complete a Declination form. • Splitting Premiums: • If you are “splitting” the premiums with your spouse that is employed by another district and the coverage is under their social security number, you must complete a declination form. However, you do not have to complete a new Split Premium form unless one of you has changed districts. • Both Birdville Employees: • If you and your spouse are both Birdville employees and are enrolled as a family under one employee, the employee that does not have the deduction taken from their check must complete the declination form

Explanation of BenefitsEOB • EOB statements available online through Blue Access for Members (BAM) • When your TRS-ActiveCare medical benefit claims are processed by Blue Cross and Blue Shield of Texas, your explanation of benefits (EOB) statements will be delivered electronically through BAM unless you specifically request to continue receiving paper EOBs in the mail. The EOB outlines the disposition of your claim, including the amount paid. With electronic EOBs, you can choose to receive an e-mail notification to the e-mail address designated by you whenever one of your claims is processed. You may then log into BAM to see the EOB, review 18 months of EOB history, see at-a-glance claim status and print copies for your records, if needed. • To register for BAM, have your Blue Cross and Blue Shield of Texas/TRS-ActiveCare ID card handy and go to the Web site, www.bcbstx.com/trs. Click on the log in link under Blue Access for Members, then select the Register Now button under the link for New User. Our group # is 085000 and use your ssn for the ID #. • If you must receive your EOBs through the mail, call TRS-ActiveCare customer service at 1-866-355-5999.

Sick Leave Bank • Sick Leave Bank • If you are not a member of the sick bank, you may join during open enrollment. • Not sure you are a member? • Follow instructions to view your paycheck (slide12). Scroll down to Leave Balances • If Sick Bank 0.00 is listed, you are a member and do not need to join again. • If not, complete a sick bank form and return to Babs Holly. Special Enrollment EventFamily Status Change Making a change during NON open enrollment • Special Enrollment event/family status change • Marriage, divorce (resulting in a loss of coverage), • Birth, adoption or placement for adoption, or if an individual with other health insurance coverage involuntarily loses that coverage • Email or call the Benefits office if you think you have a family status change • You have 30 days from the qualifying event date to notify the business office that you have a family status change

Access to Employee Benefits HUB • BISD web page>STAFF>under Quick Links> Employee Benefits HUB> OR www.birdvilleisdbenefits.com • Benefits Log-in: • Username: First six letters of last name + first letter of first name + last four digits of SSN • Password: Full last name + last 4 digits of SSN • Personal Information – change password, etc • Dependent Information - to add dependents and/or social security numbers • Benefits Plan Information • Benefit Plan Enrollment – to make plan changes or elect additional benefits • Consolidated Enrollment Plan – to review current plans – print a copy for your records, do not send a copy of Benefits office • Change Beneficiary Please review your beneficiary elections
