  11. 1. Gather requirements of the OSA, which are: Activity form – new system: reserved via online at E-RESERVE. The form is needed to be encoded by Kuya Jake, from the CFAD office to place authentication that this activity is part of our curriculum. Cover letter – aka the intro letter that says who, what, when, for and why of the activity Project proposal – added information for the admin In case they wants to dig deeper into the project For In-Campus Activity process:

  12. 2, Have the papers (the requirements) signed by the CFAD Officials, “The Big 4 plus one”. 3. Send all signed requirements back to OSA, as to be approved by Ma’am Songco. 4. If the activity is approved, go to FMO, Facilities and Management Office as to ask requests for set-up staff and other assistance. 5. The approved letter by the Sec Gen is followed by the approval of the Vice Rector of Finance. We wait for them to return the papers. 6. After all of these are signed and approved, photocopies of the forms are to be given to committees as proof of approval.

  14. The same process follows with additional inserts and additional such as waiver, map/location site, model workshops and the event itself. For Off-Campus Activity process:

  15. Aside from the In-Campus and Off-Campus forms, there are also forms for Posting. This is in collaboration with the Creatives and Promotions team. We ask for posting forms from the Sec Gen’s office, fill it up, and have it signed by Dean Loza (together with the posters). If approved, we go back to Sec Gen to have it signed officially. (more or less 3 days before we can claim it for each of the posters are to be signed). Other Papers

  17. Download. All format files are uploaded at our YGroup: http://groups.yahoo.com/group/IAC2011/

  18. Collect. Gather datas/infos from target group: FS Executives, Committees, Students, etc.

  19. Encode. Type gathered datas/infos on provided format files: Microsoft Office – Excel / Word

  20. Submit. Submit all finished encoded files at our IAC YGroup.

  22. RESPOND. When we CONTACT you, please REPLY.

  23. COMMUNICATE. If you have question, suggestion, problems, in need of help, even violent reactions – talk to us.

  24. COOPERATE. When a task is assigned to you, please cooperate and get the job done.

  26. MEET THE DEADLINES. Always meet the deadline, finish the task on or much better before the deadline.
