  2. Advance Medical Directives • Health Care Power of Attorney • Living Will • DNRCC(Do Not Resuscitate Comfort Care)

Health Care Power of Attorney Delegates power to make medical decisions to the person (agent) of your choosing, even if you are only temporarily unable to communicate Agent’s powers include: Admitting you to the hospital or other care center Approving/disapproving medical treatment Discontinuing artificially supplied nutrition and hydration, if consistent with your Living Will Advance Medical Directives

Advance Medical Directives • Living Will • Clarifies your wish that artificial life-sustaining treatment (including nutrition and hydration) be withheld or withdrawn if you are terminally ill and unable to communicate • Standard: “If two doctors say that you are a goner, and the nutrition and hydration are NOT providing you with comfort”

Advance Medical Directives • DNRCC (Do Not Resuscitate Comfort Care) • Concise form that specifies that Emergency Medical staff are to do the following: • WILL: Suction airway, Administer oxygen, Position for comfort, Splint or immobilize, Control bleeding, Provide pain medication & Emotional support, Contact other appropriate health care providers such as Hospice, Home Health, attending Physician • WILL NOT: Administer chest compressions, Insert artificial airway, Administer resuscitative drugs, Defibrillate or cardiovert, Provide respiratory assistance, or Cardiac monitoring • Patient selects when comfort care protocol is to be implemented • Check “DNRCC” – immediate CC • Check “DNRCC-Arrest” – CC only implemented in the event of cardiac or respiratory arrest • Certification of DNRCC status by physician

  6. Durable General Power of Attorney • Typically used when client is debilitated or physically unavailable • Delegates power to make financial decisions to an Agent • Can be effective immediately, or be a springing power • Critical tool for proper Medicaid planning
