CAUTI Rates (Outcome) • Collected and submitted for the entire month • Use NHSN CAUTI definitions (see Resources slides) • CAUTIs ascribed to the unit – total number of symptomatic CAUTIs ascribed to the unit during the month • Urinary Catheter Days – total number of patients in the unit with an indwelling urinary catheter device during the month • Collected at the same time each day, then daily counts are summed and the total is entered into Care Counts • Patient Days – total number of patients in the unit during the month • Collected at the same time each day, then daily counts are summed and the total is entered into Care Counts • Resulting data • CAUTI Rates • Catheter Prevalence Important: Dates in this presentation apply only to units in Cohort 7, beginning data collection in January 2014

CAUTI Rates (Outcome) • Three phases of data collection • Baseline is collected for three consecutive months • January, February, and March 2014 • Implementation is collected for two consecutive months • April, and May 2014 • Sustainability is collected one month of each quarter • August 2014, November 2014, February 2015, May 2015 • Entering data into MHA Care Counts • Cannot be entered until the data collection month ends • Example: January 2014 data can be entered on February 1, 2014 • Due by the end of the following month • Example: January 2014 data is due on February 28, 2014 Important: Dates in this presentation apply only to units in Cohort 7, beginning data collection in January 2014

CAUTI Rates (Outcome) • Importing from National Healthcare Safety Network (NHSN) • Same unit in this project also submitting CAUTI Rates data to NHSN? • State Lead may offer to import this data on your behalf • Eliminates the need to manually enter data • Requires conferral of rights in NHSN to your State Lead’s organization • Contact your State Lead for more information • Only available for CAUTI Rates (Outcome) data • Prevalence & Appropriateness (Process) and Team Checkup Tool data not collected in NHSN Important: Dates in this presentation apply only to units in Cohort 7, beginning data collection in January 2014

Prevalence & Appropriateness (Process) • Collected and submitted for each day • Number of patients on the unit that day • Number of catheterized patients on the unit that day • Main reason why patient has a catheter that day • Use HICPAC Guidelines for Appropriate Indications (see Resources slides) • Where the catheter was inserted • On floor / Off floor / Unknown • Printable form that may assist your unit in collecting Prevalence & Appropriateness (Process) data available (see Resources slides) • Resulting data • Catheter appropriateness Important: Dates in this presentation apply only to units in Cohort 7, beginning data collection in January 2014

Team Checkup Tool (TCT) • Completed once at the end of each quarter to assess unit’s progress that quarter • Unit’s implementation of recommended CUSP activities and CAUTI activities • Team interactions and functioning • Barriers to progress • Recommended way of completing • Unit team leader prints a copy of the TCT • Printable Team Checkup Tool available (see Resources slides) • Team meets, discusses and agrees to all answers • One person enters the team’s answers into Care Counts Important: Dates in this presentation apply only to units in Cohort 7, beginning data collection in January 2014

Team Checkup Tool (TCT) • Entering data into MHA Care Counts • Cannot be entered until the data collection quarter ends • Example: 1st Quarter 2014 TCT can be completed on April 1, 2014 • 1st Quarter is considered January, February, and March • Due by the end of the first month after the quarter ends • Example: 1st Quarter 2014 TCT is due on April 30, 2014 • Important to complete as timely as possible • “Snapshot” in time of unit’s progress Important: Dates in this presentation apply only to units in Cohort 7, beginning data collection in January 2014
