CENTRAL LINE CHAIN OF INFECTION Break the Chain Bacteremia of Catheter & Peripheral Blood Bacteremia of Catheter Lumen /Site Preventions Hand Hygiene/Cap change with tubing, blood & TPN/Scrub the Hub/CHG skin prep/Site selection/Consolidate catheter entries/DC line if not needed/Bundle supplies/Barrier precautions Treatment ABX Therapy based on BCX recovery Sepsis Site Infection Disseminated Colonization Colonization of Catheter Lumen/Site Disseminated Infection Colonization

  2. HAND HYGIENE Blood Draws Dressing Change Cap Change Etc. Dumping Hats Empting Foleys Etc.

CAP CARE & CHANGE • Cap Care • Scrub the cap for 15 seconds. • Allow to dry for 15 seconds. • If blood on cap after draw, scrub for 15 another seconds. • Perform hand hygiene before downing gloves. • Site Scrub with cap changes – scrub hub 10 sec, dry for 5 sec • Cap Change • Change caps & tubing every Tue/Fri or 96 hrs. • Change caps when: • TPN is changed. • After administering PRBC’s. • Before drawing blood cultures. • After administering lipid containing solutions. • Document in Power Chart.

CENTRAL LINE DRESSINGS Dressing Change • Every 7 days • >25% of edges lifting = Now • Damp or visibly soiled= Now • Mask on patient and nurse • Clean gloves during removal • Sterile gloves during cleaning and placement • Use chlorohexidine-impregnated sponge to disinfect. Inspect insertion site q 4 hrs.
