Clerking Check List for Neck of Femur FracturesFor orthopaedic FY1/2 or CT1/2 Patient Name: Hospital No: Date : Time : • Check adequate Analgesia Prescribed and Administered + Pain score documented. • Identify and treat correctable co-morbidities immediately so that surgery is not delayed by: • AMT on all patients • Ensure IVF started (unless very severe HF) • Check ECG + CXR + Hip Xray done in ED and document findings • Check U&E, FBC and Coagulation Screen taken in ED and document findings • Group and save +/- cross match (check if confirmatory sample needed) • Check what time patient last ate ….. : ….. and drank ….. : ….. • NBM (if having operation same day) • Does the patient need pre-operative investigations? • Pacemaker check (if not within the last year) • If clinical indication for ECHO d/w anaesthetist first • Online VTE assessment + prophylactic tinzaparin @ 18.00 (unless contraindicated) • Consent patient • Mark side with permanent marker • Add patients details to • Trauma list in Theatres • Trauma board in Thorogood Seminar Room • Check Waterlow score done by nursing staff • Check pressure-relieving mattress and bed on COYLE WARD has been requested by ED • Bleep anaesthetist 3005 without delay and inform them of the patients whereabouts, relevant medical history and current medical condition.
