  1. VAP • Wash hands and wear gloves with all patient contact • Head of bed elevated as tolerated • Use oral suction device, keep in clean container between use and change q24 • Do not store in plastic bag • Use inline suction only and change q24 • Avoid using wet nebulizer treatments and avoid disconnection from ventilator • If disconnecting from ventilator cover open end with a clean plug or sterile package

  2. VAP • Avoid water dumps from Ventilator circuits • Plan to switch to the new humidifier with the upgraded circuit in 2006. • Assess readiness for extubation daily in • Assess readiness for feeding daily • Assess the need for OG/NG tube daily if not ready to feed. • Zantac is added to the HA when clinical indicated • All ambu bags are stored in a plastic bag at the bedside • All intubated patients have an HME on their bag. • HME are changed q24
