Missed opportunities for skin inspection and early detection of risk & early implementation Lack of appropriate care planning and ownership by Registered Nurses Lack of equipment available in certain areas Staffing issues and reliance on temporary staff Delay or absence of TVN review of Grade 2 PU Delayed roll out of new PAM documentation
