  4. Step 1: select the correct requisition Requisitions available through standard register: 15-125040 general surgical pathology 15-121100A gyn 15-121100B placenta 15-125020 renal 0860 breast 0867 GI 0868 eye pathology dermatopathology

  5. Step 2: Complete Required Information • Patient’s full name: Last name, first name middle initial • Complete date of birth or JHH medical record number (also called history number) • Sex • Clinic location (e.g. 109, WM205, 223, etc…) • Procedure date and time (if applicable) specimen was collected *note collection time is required for breast specimens • Attending /requesting physician: full name and NPI# • Full name and address of other physicians who need a copy of the report

  6. Clinical Information • Clinical Description/ Prior History/ Working Diagnosis • Number of specimens (containers) *Specimen description on the requisition must match exactly as labeled on the corresponding specimen container
