  5. Blue Cross/Blue Shield • Choose any doctor you like • Annual out of pocket limits • Annual deductable • Pays 80% to 100%

  6. Fairhaven Centers • Complete health care in one convenient location • Simple approval for treatments • Annual out of pocket limt • Pays 90%

  7. Invest in Your Retirement • You’re eligible to contribute after 6 months of employment • Choice of 12 funds • Company matches 50% after one year

  8. Life Insurence • Salaried employees receive free life insurence worth 2 times annual salary • Additional life insurance available • Life insurence for spouse
