ACCOUNT OPENING • DOCUMENTS REQUIRED • All Employees upon joining need to Open a Zero Balance Salary Account with State Bank of India; • Employees have to provide the following documents – • ID Proof, • Address Proof & • Two Passport Size- Colored Photographs • For – ID/Address Proof– one can submit either of the – • PAN CARD/VOTER ID/PASSPORT COPY/AADHAR CARD, DRIVING LICENSE ETC. • Please contact Ms. Sangeeta Arya • sangeeta.khurana@ilfstechnologies.com

  3. MEAL CARD/VOUCHER • Meal Card/Voucher is proposed to be provided through Paytm Food Wallet. • All payroll employees are eligible to Meal Card/Voucher worth Rs. 2,000/- per month. Trainees/Consultants shall not be entitled as per Company Policy • Employees will get the Meal Coupons/Voucher, uploaded in the PayTM Wallet each month. New Joinees will start getting the same by next month after joining the ITL . Topup will be from the Date of Joining for New Joiners • Meal Voucher Amount shall be adjusted pro-rata, in case of Negative leave balance intimated by HR. • Eligible Employees will be required to Download PayTM APP on the Smart Phone and shall update that Mobile no only in TMS.

  4. Medical Coverage ITL extends a variety of reimbursements and insurance policies for the welfare and benefit of the employees to cater to their health related requirements. Keeping the well being of the employee and their dependents in mind ITL provides the following benefits: a. Group Medisheild Policy (GMC) b. Group Personal Accident Policy (GPA) c. Group Term Life Insurance Policy (GTL)

  5. Group Medi-claim Policy All employees and their dependents, subject to a maximum size of six persons will be covered under this policy with effect from the date of the employee’s joining. Definition of Dependent: Employee + Spouse and 2 dependent children + / 2 Dependant Parents or Parents in Laws. Overall mediclaim limit is as per the Graded system as mentioned in next slide. Note: Parents / parents in laws Medical Insurance premium will be borne by employee.

  7. AUTHORIZATION PROCESS- EMERGENCY (FOR CLAIM & Admission)

  8. In Emergency - Hospitalization • Road Accident, Stroke, etc.. • Call HDFC-ERGO contact centre for nearest hospital. • Provide Medical card No. along with your Emp. Photo ID Card at the Reception or the TPA/ Insurance desk. • The Insurance desk will get the pre-authorization form filled and faxed or emailed to Max Bupa Health Insurance Co Ltd. • Max Bupawill reply to the hospital within 1hr on receiving the fax with an initial approval, full payment is done at the time of discharge. • In Case Hospital ask for some advance payment for starting the treatment in case you can not wait for receiving the approval owing to medical urgency

Hospital will return the advance payment once it receives approval letter from Max Bupa. • In Case of Planned - Hospitalization • Maternity, Cataract, etc.. • E-mail Max Bupaas per contact nos./email_id given • Get the treatment pre-authorised through TPA Desk of Concerned Hospital from Max Bupa • Remaining Process remaining the same

  10. REIMBURSEMENT PROCESS

Steps to follow for Reimbursement Claim • Intimate Max Bupa about the Hospitalization at least 72 hours prior but not more than 24 hours after the hospitalization but prior to discharge. • Get admitted in the hospital nursing home of your choice. • Ensure to collect all original documents (including discharge summary, hospital bills, investigation reports, prescription etc.) in original & in proper format. • Submit claim documents in original along with claim form & cancelled cheque copy. • Claim status/query will be intimated via Email or letter by Max Bupa claim Team.

General Documents • Duly filled and signed Claim Form with Photocopy of ID card In-patient Treatment /Day Care Procedures • Original detailed discharge summary / day care summary from the hospital • Original consolidated hospital bill with break up of each Item, duly signed by the insured • Original payment receipt of the hospital bill • First consultation letter and subsequent prescriptions • Original bills, original payment receipts and reports for investigation • Original medicine bills and receipts with corresponding prescriptions Road Traffic Accident (In addition to the In-patient Treatment documents) • Copy of the first information report from police department / copy of the medico-legal certificate • Treating doctor’s certificate giving details of injuries (How, when and where injury sustained)including whetherClaimant was under the influence of any intoxicating material. For Death Cases : (In addition to the In-patient Treatment documents) • Original death summary from the hospital and Copy of the death certificate from treating doctor

Pre- and Post-hospitalisation expenses (with copy of discharge summary) • Filled claim form of Max Bupa • Original medicine bills, original payment receipt with prescriptions • Original investigations bills, original payment receipt with prescriptions and report • Original consultation bills, original payment receipt with prescription • Cancelled Cheque copy Ambulance Benefit: • Original bill with original payment receipt • Treating doctor’s consultation prescription indicating emergency hospitalization

  14. Group Personal Accident Policy The Company has availed a Group Personal Accident (GPA) Policy coverage from a Life Insurer for its employees for providing adequate financial coverage, in the event of an unfortunate “Partial or Total Disability (both temporary & confirmed)" or "Death" of the employee concerned. The aggregate maximum Coverage is up to five (5) times of the Annual CTC of the Employee at the time of joining the Company or at the time of renewal of policy as the case may be. Group Term Life Cover Policy The Company has availed Life Insurance Policy coverage from a Life Insurer for its employees. Under this policy the company will pay a Death Benefit in the unfortunate event of death of an employee. The aggregate maximum coverage is up to three (3) times of the Annual CTC of the employee at the time of joining the company or at the time of renewal of policy, subject to a minimum of Rs.5 lacs as the case may be.
