Eitas – Empowering Individuals Through Advocacy and Support • County Board – Support Coordination, funding for services and transportation • 55 SC’s and growing-hire a new team each year – we serve over 2000 people and their families • Urban Core and Rural service area • 72 residential providers and over 100 Waiver Service providers – Serve more people receiving 24 hour supports than St. Louis.

Timeline State and Federal EITAS • HCBS Rule – 2014 • 2015 • New Missouri Quality Outcomes • MO submits Transition Plan for HCBS Rule • Remediation required ISPs related to HCBS rule • Missouri stakeholders start developing new ISP Guide 2015-2016 LifeCourse used widely in Missouri and growing in the US • New Individualized Support Plan Guide – MO 2017 • 2017 LifeCourse Showcase in KC! • 2013 - Life Course…we are hearing about LifeCourse-that is for families…right?? • 2015 - Wow! LifeCourse Framework is now a part of our state quality of life measures – MO Quality Outcomes • 2015-Eitas starts training new Support Coordinators in LifeCourse principles and tools & Person Centered Thinking. Lots of consultation with Sheli and team • 2016 -38 Support Coordinators/leaders attend LifeCourse Community Event -2016 sponsored by Division of DD • 2017 - EITAS develops new ISP and organization wide training

  4. Missouri Quality Outcomes In 2015 Missouri updated the Missouri Quality Outcomes. There are 7 areas of MO Quality Outcomes. Missouri intentionally integrated the LifeCourse Domains into the MO Quality Outcomes. The state is using the new MO Quality Outcomes to measure quality of life for Missourians with developmental disabilities.

  5. ISP Guide – Missouri MO Quality Outcomes used to guide the revised ISP Guide implemented January of 2017. The guide is divided into sections that make it easy to utilize person centered planning processes including: Vision for a Good Life and Important To Sections of the ISP are organized by MO Quality Outcomes = LifeCourse Domains

Opportunities from Support Coordination Organization perspective – System Oriented • Not getting to do the real work that feeds the soul • Overwhelmed with paper, regulations, monitoring responsibilities • Losing the essence of person centered planning and support and advocacy • LifeCourse sets the direction of our work to achieve our mission • LifeCourse - is engaging, practical – can use tomorrow • LifeCourse principles can be integrated into processes, systems and our day to day thinking organization wide

New ISP format - Opportunities • Get to “do it right” within the system • Bring it all together so it makes sense to SC’s • Person Centered Planning Process and OUTCOMES!!!!!! • Individualized Support Plan format incorporates LifeCourse and the intent of the HCBS rule • Path of least resistance

  8. Eitas Process Person Centered Support Plan Pilot - 10th draft Workgroup comprised of Support Coordinators with a range of experience with LifeCourse and Person Centered Planning. Discussed the pros/cons of current and what will be the pro/cons of the new template/format People doing the work directed the work; people we support provided input and providers

  9. New ISP – 3parts • Person Centered Planning process • embedded into the first several pages • Personal Outcomes Moved closer to the front document • Support Information • Based on LC Domains • Addresses the bulk of the details of required information for Missouri DMH ISP Guide

Training • SC’s do own portfolio • Simple exercises to understand how to apply the principles to get information • Using interactive, group activities to help people understand we use the tools in our planning process to cover the LC domains • To help with overwhelmed feeling – use the next chart to identify “low hanging fruit” opportunities and to strategize best approaches/expectations

  11. LifeCourse Domains LifeCourse connects to all parts of a person. We use this graphic in training with youth planning for transition. Soon we will use the graphic

  12. Personal Outcomes Move away from “Service Life” outcomes that reinforce a “green only” life. Person First Outcomes – colorful life!
