A4 New Tax Form Requirement Marilyn Mulhollan Executive Director, Financial Services
All ASU employees (including faculty, staff, graduate and research assistants and student workers) who will receive a paycheck from ASU after June 30, 2010, must file a new Employee’s Arizona Withholding Percentage Election (Arizona Form A-4). The new form is available for electronic filing through MyASU or in paper form on the ASU HR Forms website. If an employee has not filed an A-4 form electronically, it will show as a needed action on their MyASU/My Tasks/To Do list. The day after the form has been electronically filed, it should no longer appear on the employee’s To Do list.The electronic form should be completed by all employees who have access to the internet and MyASU. The new withholding election must be submitted by June 15th for the change to be in effect for the paycheck issued on July 2nd. Failure to submit a new A-4 form will result in 2.7% of gross taxable wages being withheld, until a new form has been submitted. The new form can be submitted now and it will be effective on the July 2, 2010 paycheck.This is a State of Arizona mandatory requirement and impacts employees of private businesses as well as government agencies. Filing the new form is necessary in order to comply with a change being made to the method of calculating Arizona income tax withholding for salary/wages paid after June 30, 2010. Currently, Arizona state income taxes are withheld based on a percentage of federal income tax withheld. The new mandate is to withhold Arizona state income taxes based on a percentage of gross taxable wages.
For assistance in calculating the percentage to withhold, the State of Arizona has provided a worksheet on the second page of the paper A-4 form. (The worksheet is for the employee’s records and should not be submitted.) The State has also provided the conversion table shown below. Your current Arizona withholding percentage is listed on your paycheck in the Tax Data section as AZ Pct Fed W/H. To see previous paychecks, go to MyASU /My Employment/Payroll/View Paycheck. ASU Financial Services cannot provide tax advice. The worksheet and table tools may help with the selection process, but employees should consult tax professionals if they have specific questions about the most appropriate withholding rate to select.
