4. (C1) Select Category that Applies (1) Choose “Select” if your name appears here after entering your ID number above.

5. (C2) Select Category that Applies Select “This is a New Worker” after entering your ID number if no matching records are found.

6. (D) Enter Information Needed (2) Must select PI when finished above (1) Most info will auto-populate if worker is in ASU HR system. (2) If new worker please complete information required & designated by Required

7. (E) Select Affiliated PI or PIs (2) Select “OK” when finished (1) Select all PI’s with whom you are associated

8. (F) Submit Information (2) Select to choose Job Code & Description (1)“PI” information auto populates (3) Select when finished

9. (G) Finished for Existing Worker Select when finished
