12. WHENmust a report bemade? • Class A disease –immediatelybyphone • Class B disease – by the end of thenext businessday • Class C disease – by the end of thenext businessday

WHERE must the report bemade? • Healthcare providers and laboratorians should report to the local health jurisdiction in which the case or suspected caseresides • If the residence is unknown, report to the local health jurisdiction in which the healthcare provider or laboratory islocated • If the residence is outside of Ohio, report to the state of patientresidence

HOWmust a report bemade? • Class A – immediately by phone, follow-up with HEA 3333 as instructed by health jurisdiction; follow up can also be done through direct entry into Ohio Disease Reporting System(ODRS) • Class B and C – HEA 3333; Class B and C can alsobe • done through direct entry intoODRS Labs that report 200 or more Class A,B or C diseases each year should be working towards Electronic Laboratory Reporting (ELR)
