  4. Why Are Chronic Diseases (CDs) Such A Big Problem? • Chronic diseases [heart disease, diabetes, cancer, etc.] • 7 of 10 deaths US; 6 of 10 world-wide; China 8 of 10 • 75% of U.S. health care expenditures (HCE) are for CD • In 2009 HCE 17.6% of Gross Domestic Product (GDP) • In 2019 HCE 19.3% (GDP) https://www.cms.gov/NationalHealthExpendData/25_NHE_Fact_sheet.asp • In 2035 HCE 31% (GDP) http://www.cbo.gov/ftpdocs/102xx/doc10297/Chapter2.5.1.shtml • In 2040 HCE 34 % (GDP) http://www.whitehouse.gov/assets/documents/CEA_Health_Care_Report.pdf • In 2080 HCE 46% (GDP) http://www.cbo.gov/ftpdocs/102xx/doc10297/Chapter2.5.1.shtml

  5. Causes of CDs • WHO risk factors – poor diet, physical inactivity, tobacco use • CDC (U.S.) risk factors – poor diet, physical inactivity, tobacco & alcohol use

  6. Can We “Beat” Chronic Diseases? • WHO Says – “if the major risk factors for chronic disease were eliminated at least 80% of heart disease, stroke and type 2 diabetes would be prevented and 40% of cancers” U.S. Dietary Patterns

  7. Causes – U.S. & World-Wide • Urbanization – westernization • Processed foods, manufacturing, marketing, transportation • Increased meats; added fats & oils; calories sweeteners; decreased total grains - increased refined grains; (U.S. cheese increase) • Low nutrient density & high caloric density • Increased calories > 500 cal per day 1970-2000 • Reduced physical activity

  8. Real Causes of Chronic Disease? …U.S. Dietary Patterns Over The Last 100 Years…

  9. Source: USDA/Economic Research Service, Food Availability Data

  10. Results of U.S. Dietary Patterns • 500 extra calories per day from 1970-2000 • 3500 calories per week = 1 pound per week! • Low nutrient density • High caloric density • Low fiber and bulk • Increased in bad fats • > 70% of U.S. population is overweight • ↑calories + ↓nutrients = ↑inflammation = ↑in CHRONIC DISEASES

  11. What Drives Chronic Diseases In U.S. Besides Bad Lifestyle Habits?

  12. Our ‘Disease-Care’Model • Incentives - let disease happen; treat it; then pay for it! • No incentives to prevent disease – none! • Capitalism is about selling as much as you can whether it is good or bad for you! (but capitalism can be the solution too…) • Truth – 1 of anything won’t harm you BUT…Think of all the food ad $$$$...Its not about selling one of anything!

  13. Who Pays for Your Bad Habits? • Small business example – my workplace • Health premiums increasing 10-20% / yr • My health premium age 54 - $590 • At age 55 - $725 • I do not have a chronic disease • On no medication • Who am I paying for? Others my age with CDs that are preventable and reversible! • Business switched to Kaiser • We are all in this together

  14. “New Attitude” • When we build a new hospital, cancer or transplant center we fail! • Needing more nurses is failure! • Needing more specialists is a failure and prescription for bankrupting our economy • Donating to a society over and over again for a chronic disease (heart, diabetes, etc.) that says “Take your medicine, watch your diet and donate to us” is going to bankrupt us and make people sicker! • Relying on health insurance makes us sicker not healthier!

  15. Goals of Healthcare Reform • Educate the public on what are chronic diseases (CDs) • Educate the public on the societal impact of CDs • Educate the public on the lifestyle causes of CDs (5 dietary patterns, inactivity) • Educate the public that CDs are preventable and reversible by diet and exercise. • Prevent and reverse chronic disease • Create incentives for health professionals to keep people well. • Create incentives (or penalties) for people to stay well. • Preserve the environment and ecosystems • Reduce unnecessary human and animal suffering

  16. NUTRITION CONCEPTS

  17. Diet Basics -Everyone Agrees • Unprocessed • High in nutrient density • Low caloric density (CD) • Controls the glycemic response • Reduces insulin • Anti-inflammatory • Lots of vegetables
