  2. Basic Life Eligibility Basic life insurance coverage is provided by Uintah School District at no cost to: • Benefited 20-40 hour per week employees • Employee’s Spouse • Employee’s Dependent Child(ren) (Each eligible dependent child must be unmarried and under the age of 26)

  3. Basic Life Benefit Amounts • Employee: $50,000 • Spouse: $2,000 • Dependent Child(ren): $2,000

  4. Supplemental Life Guaranteed Issue Amount The Guaranteed Issue Amount is the amount of insurance that you elect without providing evidence of good health. The Guaranteed Issue Amount applies to new hires who apply for coverage within 31 days of their hire date. The GIA amounts are: • Employee: $100,000 • Spouse: $25,000 • If you enroll after the new hire period, or elect an increase in coverage, evidence of good health will be required for all coverage amounts.

  5. Supplemental Life Benefit Amount • Employee: Option to purchase coverage in increments of $5,000, from a minimum of $20,000 to a maximum of $500,000. • Spouse: Increments of $5,000, from a minimum of $10,000 to a maximum of $250,000 (not to exceed Employee Supplemental Life amount). • Child(ren): If you apply for a minimum of $20,000 on the Employee Supplemental Life plan, you may apply for supplemental coverage on your dependent child(ren) for either $5,000 or $10,000 (maximum). Child benefit not subject to providing evidence of good health.

  6. Benefit Reductions & Portability • All coverage amounts are reduced beginning at age 65. Benefits terminate upon retirement. • If you leave your employment with Uintah School District , Portability is an option that allows you to continue your Supplemental Life Insurance coverage. To be eligible you must terminate your employment prior to age 65 and you must be porting a minimum of $5,000. You may port a maximum of $250,000.

  7. Travel Assist • The Travel Assist program provides you, your Spouse and covered Dependent Children with immediate access to doctors, medical facilities and certain other travel-related services when faced with a medical emergency while traveling 100 miles or more from home or in a foreign country. Please contact the Business Office for a Travel Assist brochure for further plan information.

  8. Living Benefits Option • Should you be diagnosed as terminally ill with a life expectancy of 12 months or less, the Living Benefits option allows you to receive an accelerated payment of a portion of your group life insurance. You may request a minimum accelerated payment of $3,00 up to a maximum of 80% of your coverage not to exceed $500,000. Funds are paid directly to you, with no policy restrictions on how you use them. The remaining benefit is then payable to your beneficiary upon your death.

  9. Waiver of Premium • This provision applies if you become Disabled before age 60 and your Disability lasts for at least 6 months. You must provide proof of your condition within one year of your last day of work. Once approved, your coverage will continue without payment of premium up to normal retirement age, as long as you remain disabled.
