  12. RE-TRAINING • ASSIGNED TO DIFFERENT TYPE OF TRUCK • EVALUATE OPERATOR’S PERFORMANCE EVERY 3 YEARS • Refresher course every 3 years (not required)

  13. CERTIFICATION • EMPLOYER MUST CERTIFY THAT EACH EQUIPMENT OPERATOR HAS BEEN PROPERLY TRAINED AND EVALUATED
