  2. Definition of a major accident There are several definitions, we have chosen the most common two. The first describe extent, the other causes. • An accident(a sudden indicent with loss) where at least 5 people are exposed • An accident caused by error on one or several safety/emergency systems

  3. Feedback from CAKE Q2 shows that despite our high HSE focus we still have silent deviations and situations when people don’t say stop. The main reason seems to be that it is easier to focus on the immediate consequences than those far ahead and on possible chain reactions. Mainly silent deviations within: • barrier/red zone • working in heights • PPE when working with chemicals • WP (wrong use, missing WP, area not check before starting the job) • lifting Main situations when you don’t say stop: • avoid conflict • fear of consequences (management, team, career) • timepressure (also self-imposed) • culture (always done it like this, it’s okay, goes faster, aksept for shortcuts etc) • based on experience (the person doing the job knows best)

To optimize the Stop work directive, following success criterias where mentioned: From management: • regularly inform and encourage use of the directive • accept and acknowledge when someone says stop From crew: • don’t accept silent deviation • back up the person saying stop • be engaged in TBT • be open for and respect new/other opinions From the individual: • think for yourself • speak up • use VO as support if needed • accept when someone says stop • Constant care!

  5. To increase understanding of personal risk and consequences, last part of CAKE Q3 will have a practical assignment for each department. In this assignment a ”worst case scenario” for that department will be discussed, and how different silent deviations and nobody saying stop may affect course of events towards a major accident.The purpose is visualization of my responsibility in a chain reaction, for instance when I take part in a poor TBT or ignore to say stop.

  6. Whether you are at work or at home – keep the movie in the back of yout head. Have you seen anything that should have been fixed – do not postpone it! Do you feel a timepressure – ”take two” and discuss with leader/collegue!
