Background • The Sheltered Housing Service was transferred from Housing to Social Care in September 2005 • In May 2013 there was a realignment of the service (patch based community support service and emergency response team merged with the Community Based Prevention Services and the Dudley Telecare Service) • 11 Schemes remain across the borough providing 390 tenancies, 2 intermediate care support flats and 5 Scheme Managers still live on site, covering Monday to Friday

  5. Issues • Appraise the original re-alignment options for supported housing report • Stock fit for purpose • Fire risk assessments • Two Extra Care Schemes • Sheltered Housing to be a real alternative to residential care

Proposals • Determine the future of Sheltered Housing to reflect the current strategic drivers within the Directorate • Agree the scope of an independent review, to be carried out across the 11 Sheltered Housing Schemes • Define a set of minimum living standards for tenancies within the 11 schemes in the absence of decent home requirements • Agree for Buildings services to carry out an audit of assets and capital investment requirements to ensure the housing stock in fit for purpose

  8. Approval to Procure • Specialist consultant to carry out an independent review of the 11 Sheltered Housing Schemes across the borough • Internal Building Services to carry out an asset review in terms of the stock

Scope of the External Review • Provide good examples of sheltered housing nationally and a comparable to Dudley’s model, combined with gaps in provision • Identify any sheltered housing stock that is not fit for purpose and provide a recommendation for a more effective, alternate use, linking to wider social care • Identify recommendations and cost implications, for two of the schemes which have the potential to be housing with care • Evaluate the cost benefits of alternative models of care through personal budgets, providing greater choice and control. The care within two schemes is provided internal home care and sleep-ins covered by staff in housing • Review the staffing structure that covers 5 days, in office hours, including the benefit of Scheme Managers living on site compared to attending the scheme

External Review - RECOMMENDATIONS • New Sheltered Housing Model – Developing and implementing a new resource structure focusing on the role of health and well-being • Training and Awareness – Creating and implementation strategy to increase awareness of the sheltered housing service and the further roll out of existing training programs • Allocation and Review Process – Simple changes to improve the existing process and reflect the strategic approach to managing needs • Activities – Development and implementation of a consistent activities program across all schemes building on links with internal council led initiatives, external organisation and existing volunteer networks • Building Improvement Program – Agreement of building standards and the implementation of an improvement program to enable better utilisation of existing resources • Care and Support – Removal of the on site staff and the existing “sleep in” service , along with the development of a dementia strategy and the review of existing performance indicators

Telecare • Traditional hardwired equipment • Alarm, pendant, pull cords and door entry system • Scheme Manager has hand held in the day • Switches over to the Dudley Telecare Service monitoring and response centre, out of hours • The Dudley Telecare Service is now part of the prevention offer and part of Living Well Feeling Safe • Bridge the Gap – Involve and Get Involved

LIVING WELL FEELING SAFE We aim to help people age well: healthy, independent, happy and feel safe and secure in their own homes By providing a range of support services, that can make a huge difference to people’s lives ensuring they can continue to live independently Mitigating Risks: Fire, Burn, Death, Distraction Burglary, Scams, Rogue Traders Falls / Injury / Not being able to call for help Personal and Emotional Safety, Getting lost, Social Isolation Non-compliance with medication, Health and Well Being and not keeping warm, hydrated

Home safety, security and home improvements • Telecare Assisted Living Technologies • Support to anyone that has served in the British Armed forces – including crisis grants, handyman services, benefits advice and respite care • Offering emotional and practical support to victims of crime • Free home fire safety checks – advice and smoke alarms • Support with bogus callers and crime disorder • Crime prevention advice and neighbourhood watch schemes • Age UK small tasks service • Supporting people with Dementia and their carers • Regular home support visits • One to one support for anyone who wants to improve their health and well being by making healthy life choices • Leisure and social activities • Falls prevention Services Provided

Data was captured on Mosaic July 2010 • Identified by Intelligence from service delivery data • obtained from partners from the previous 12 to 18 • months • Together with census information • Highlight specific areas of vulnerability with little or no • up-take of preventative services • Used the data to target hard to reach people that may • be requiring services • Audit highlighted “There is no better way of utilising • resources more effectively than to identify your target • audience and the detailed mapping should help • you to tease out which areas to be focusing on” Mapping Exercise

Partnership – go into the target areas for 3 days • Identify a community hub – centre of operations • Identify streets – target audience older people • Distribute 800 to 1000 letters • Partners identify staffing resources available and can be trained to knock doors and talk to people, raising awareness, signposting them to services • 3rd day host a Market Place – for the community • Combined with awareness session – (Who’s after your money, crime prevention, fire safety advice, • safety and security, expert patients and health and • well being advice) Programme of Work

The journey so far has taken us across the Dudley borough: • June & November 2011 • April, June & September 2012 • Greater engagement with Health and Social Care • Professionals – first event January 2013 • April, June, September plus A Darker Nights Campaign in October 2013 (Partners have key messages to raise awareness and support people at different times of year) • Practical support with electric blanket testing; advice for people how to keep warm in winter, addressing people’s concern about the cost of utilities - top tips to make savings; Police Officers will also be in attendance to display darker • night messages and provide information and advice • Health and Well Being Event October 2013 “Out and About” EVENTS

  24. My pride often gets in the way I don’t ask  Today made me feel valued & I felt I could express my needs without being judged OLD ! Good event however I need information on sight failure  I cannot believe the kindness ..at 92yrs I should have gone by now !!!!  Very useful , very impressed. Found important tips on safety  : Excellent Day x 6  Came with a friend very glad I did .  Pleased people are using our services in Hasbury  Excellent information ! Friendly professionals who I didn’t feel would whip me into a care home. Plus she drives  We will share this information we have obtained today with family & friends  BIG Thank you from us all at Hasbury Christian Fellowship. The event was excellent you used our new building for the purpose it was created for “The Community”. People have gathered plenty of information etc to keep them stronger for longer .  What I expected but I still need a gardener and have received help to locate one What about men ? I have a prostate problem  I will be sharing this information with my U3A friends 

Its critical for people of our age to keep warm  Police Ladies made us very welcome  Exercise Leaflets will be very useful .  : Wonderful Day I think I may have obtained advice about a holiday  Not only did I take away lots of useful information , I met a friend I had not seen for 19 yrs and rekindled our friendship over a cuppa  The young man who made my tea was very kind  I feel informed of what’s available and this will help me with my anxiety  What a good idea to inform people this way .  Very useful very impressed Found important tips on Someone will help me fill in a form I’ve been delaying  We’ve lost our Darby & Joan please could we have it back x 4 Information about caring will help me plan mine and my brothers future  Some useful info can this happen again ? Pill Dispenser Tracker System Wonderful didn’t have a clue they existed till today  LWFS HASBURY 2013 Trading Standards have helped me understand the law and I’m having a door chain applied  I’m going to volunteer  I need support in the early evening and at night with my wife . Still find it difficult to obtain help  Not what I expected at all ! Glad I made the effort to come I didn’t have a smoke detector. I ‘m having one now  Grants available to help me in my own home  Enjoyed bringing my mum & dad along
