APPLICATION CHANGES • OBJECTIVES: • At the conclusion of this presentation, participants should be able to: • 1.) Identify significant changes in the 7th Edition Application. • 2.) Relate changes in the application to revisions in the 2007 National Standards for DSME. • 3.) List steps in submitting a successful application. diabetes.org
APPLICATION CHANGES • Type of Application: • a.) Original • b.) Renewal (previously “extension”) • c.) Failed Re-Apply • d.) Adding sites: multi and expansion diabetes.org
APPLICATION CHANGES • ENTERING NUMBER OF SITES: • For a, b & c: If you enter a number >1 you will get a drop down menu, asking you to specify: • # of expansion sites • # of multi sites (include primary site in this number) • For d: If you enter any number, you will get same drop down diabetes.org
APPLICATION CHANGES • DATA PERIOD: • CHANGED FROM 6 MONTHS TO 3 MONTHS • MINIMUM # OF PATIENTS CHANGED FROM 15 TO 10 diabetes.org

APPLICATION CHANGES • Coordinator Credentials: • Must submit copies of licenses, registrations, certifications or degrees held. • Must be CDE, BC-ADM or have required # of CEU’s • If the DSME entity is a single discipline program, must have 20 • If the DSME entity is a multi-discipline program, must have 15 • Topics to include: chronic disease care, patient education and program management diabetes.org
APPLICATION CHANGES • ADVISORY GROUP: • a.) Document that Advisory Group consists of, at minimum: • Healthcare Professional(s) • Person(s) affected by Diabetes • Community Member(s) b.) Documentation of Input – at least annually diabetes.org

APPLICATION CHANGES • Instructor Credentials: • License, registration, certificate #’s and expiration dates • Certifications:CDE, BC-ADM, Other • CEU’s: For non-CDE/BC-ADM’s Number of CEU’s is determined by type of program per site. • Single discipline - 20 • Multi discipline - 15 diabetes.org
APPLICATION CHANGES • CURRICULUM: There is a written curriculum with learning objectives, methods of delivery and evaluation of learning for the population served, including pre-diabetes, type 1, type 2, GDM and pregnancy complicated by diabetes. • 9 CONTENT AREAS • Diabetes disease process and treatment options • Incorporating nutritional management into lifestyle • Incorporating physical activity into lifestyle diabetes.org
APPLICATION CHANGES • Medication use • Preventing, detecting and treating acute complications • Preventing, detecting and treating chronic complications • Developing personal strategies to address psychosocial issues/concerns • Developing personal strategies to promote health and behavior change diabetes.org
APPLICATION CHANGES • EDUCATION RECORDS: • Referral from a primary care provider • A comprehensive assessment • An education plan which is based on the assessed needs of the patient and includes at least one patient-selected objective • Educational interventions which include the date of intervention, content taught and name of the instructor or resource person diabetes.org
APPLICATION CHANGES • Evaluation of progress towards or achievement of learning and behavioral objectives and related outcomes • Communication with referring provider, including plan for diabetes self-management support (DSMS) DSMS is a plan, devised between the participant and the instructor, for continued support of self-management skills once they have left the program. Examples: • Return to PCP on a regular basis • Attend Support Group meetings • Attend community diabetes programs diabetes.org

APPLICATION CHANGES • EXPANSION APPLICATION: Will be available on-line • Site of origination: can be primary site or an already established multi-site. Complete all form fields. • Expansion Site: Complete all form fields. • Attestation: You must assure that all program components at the expansion site are the same as at the site of origination. • You must be able to answer all questions “yes” or the application will fail. diabetes.org
