Introduction The main objectives of this work are: • Develop a concept of solidarity; • Make an organized collection of information on the various factors responsible for the aging population; • Make an organized collection of information on the role of Geriatric Nursing; • Undertake an objective observation of the target population and the institution selected in order to characterize it. “A case study it is a methodological approach for research especially suitable when trying to understand, explore or describe events and complex contexts in which many factors are involved simultaneously” unknown

  6. Nursing and Aging The Nurse as a Health Care Professional needs to devise strategies in order to act on the determinants of active aging. The objectives of the nursing care of elderly people include dimensions such as: • Promoting autonomy, independence and self-esteem • Preventing the loss of functional skills, • Support to the family of the elderly, • Providing quality care, • Contribute to a peaceful death, among others. It is essential to evaluate physical and sensory loss, while the communication is facilitated and is demonstrated affection, thus facilitating the physical proximity of the elderly and the caregiver. !

Population’s Characteristics • Womanandmanagedequalorhigher to 65 yearsold; • Physicalandcognitivecharacteristics are affectedthroughtheageingprocess; • Elderlypopulationincreasedbynearly 1 millionindividuals; • The portuguese populationisnotdistributedequally. • Elderly are in more remoteareasofcountryside, withtheyoungerpopulationfocuses more in largecities. “Healthy aging, autonomy and independence, as long as possible, its today’s challenge to individual and collective responsibility, with translation in significant economic development of countries”(General Direction of Health, 2004).

  8. Theinstitutionselected: The Portuguese Cáritasis a Portuguese Episcopal Conference service directed for the promotion and dynamic of the social action of the church. In 1978 was created Home of Buarcos. Cáritas Network is composed by 20 diocesan Cáritas and for the local groups they work with professionals and volunteers.

Diocesan Cáritas of Coimbra Is an institution of social solidarity that supports the community in the social, educational, health and religious context. • Protect the individual dignity, based on the church principles; • Inform, denounce and sensitize the society for the social exclusion, poverty and other social emergencies.

  10. Cáritas Values Charity Humanization Professionalism Transparency Commitment These values are implemented in a “person-centered intervention, protecting his rights, freedom and warranties” (Diocesan Caritas of Coimbra, 2011).

The Cáritas of Coimbra has three nursing homes in operation: • The Home of NossaSenhora da Encarnação in Buarcos, with capacity for 106 elderly residents; • The Home of Santo António, the complex of the Headquarters and the Center of Rainha Santa Isabel, in Coimbra, for 52 elderly residents; • The Home of S. José, with the Major Seminary, with 12 priests according to residents.

  12. Requirement to Join Cáritas: Havebetween65 and 80 yearsold; Nothaving a maindiagnosisofpsychiatricdiseases. HomeofSanto António: Have to beindependent, whichmeanswalk, eatand do theirhygienecarebythemselfs.

  13. MultidisciplinaryTeam Payment Dependsonthehouseholdincomehowcorrespondsto 70% ofthe total; Thisvalue varies dependingonthedegreeofdependency; Allthecares are whithinthepriceexceptdiapersandmedication.

  14. REALITY OF A PORTUGUESE INSTITUTION • TheCáritasDiocesana of Coimbra offers support to the elderly through: • Day Center • Support Service • Elderly Residential Structure • Home “Rainha Santa Isabel” • ULDM

Day Center Provideexcellent service to the elderly population through the promotion of independence and quality of life. • Promote: • Basic hygiene and comfort; • Socializing activities (trips, cultural visits and entertainment); • Transportation between the users home and the institution; • Access to medical and pharmacy services.

  16. Elderly Residential Structure To promote independence and life quality for the elderly, through prevention, stimulation of physical, cognitive, relational and spiritual capacities Providing permanent services appropriate to the biopsychosocial problem of the elderly and developing uniform procedures as directed. Elderly Residential Structure: • Offer housing conditions for an indefinite period, • Provides hygiene, comfort and a balanced diet care • Offer permanent care to encourage the individuality, autonomy and independence of the user • Provides psychosocial support • Prevent and repair social dysfunction situations, • Offer programs of musical expression and drama, educational and cultural visits, physical and sporting activities

Home “Rainha Santa Isabel” It aims to promote independence and life quality to the elderly, through prevention and stimulation of physical, cognitive, relational and spiritual. • Provides internment for an indefinite period • Aims to preserve and encourage interfamily and • intergenerational relationship. • Provides hygiene and comfort • Provides daily nursing care • Made a periodic physiatriccontrol • Provides physiotherapeutic care

  18. Support Service This service seeks to elevate the human person, promoting at home autonomy and trying to meet the users basic needs and expectations by supporting the patient in daily life activities and giving psychosocial support, collaboration and providing access to health care.

  19. Activities • Day trips; • Themed activities like: • Grandparents Day • Family Day • Thanksgiving • Easter • Carnival… • IthasherownchapelwheretheEucharistisperformedeverySunday; The family can engage in all Theinstitutionhas a holidayhomewherethey can spend a periodof 15 daysduringthesummer, supportedbyhomehelpers.

  20. ULDM(LONG TERM AND MAINTENANCE INTERNMENT UNIT) Provide social support and health care maintenance for people with chronic diseases or processes at different levels of dependency and who do not meet conditions to be cared for at home. • Promotes their psychological health maintenance, disease prevention, rehabilitation; • Promotes medical monitorization, the prescription and drug administration; • Provides periodic physiatriccontrol; • Ensures daily nursing care, physiotherapeutic care and occupational therapy comfort.

  21. Program“Aconchego” This program seeks to combat loneliness, provide support to seniors and promote their welfare and at the same time it gives young students a home at low cost, where they can live with the "knowledge" acquired through life experience of the most old.The university students are accommodated in residences in exchange for some company and support that is necessary, since the purchase of medicines, medical follow-up. Home for whostudies, company for whoneeds.

  22. Conclusion Makingthis case studywasanasset, Cáritasisaninstitucionthataims to provideseniorsthehighestqualityoflife, trying to answertheindividualityofeachperson. Itshouldbenotedthat as the time goesbymanyneeds are beingmodified, andisthereforenecessarymeasures to supportandcareshouldbetailored to differentstagesofhumanhistory. Andtheelderly are importantelementsbecausethey are a sourceofexperience, wisdomandhelpus to addresscurrentissuesinthelifeofeachoneofus.
