  5. What Needs to Happen in Action Period 1? • Kick off the project in your practice • Strengthen your team’s QI knowledge and skills • Refine your systems approach to ADHD care

  6. If you haven’t already… • Finalize members of your core QI team • Ensure participants andnon-participants have completed the enrollment/informed consent survey • Complete the mehealth portal trainings • Review the project timeline • Determine your core QI team’s standing meeting time and tentative huddle times

  7. Checklist:Kick-Off the Project in your Practice • Finalize your list of participating physicians – double check that non-participants haven’t changed their mind! • Communicate the project goals, who will be participating, level of excitement, and potential impact at the next staff meeting and in regular communication vehicles • Display Practice KDD in office common areas, including where you will be conducting your huddles

  8. Checklist: Strengthen your team’s QI knowledge and skills • Attend monthly practice webinars/calls. Consider blocking time in your schedule to ensure your team can attend the entire call. • Keep your Model for Improvement card in an easy-to-access place and reference often. • Get in the habit of using the PDSA worksheet to document your theories, results and what you have learned. • Begin using the tools and resources provided in this session and the ADHD toolkit.

Checklist: Refine your systems approach to ADHD care • Establish roles and responsibilities of your care team • Consider updating/creating written protocols for diagnosis and follow-up visits, including roles and responsibilities of the care team • Begin entering patients into the mehealth portal • Remember, for the first cycle, the MOC requirement is an average of 5 patients per month over at least 5 months!

Checklist: Refine your systems approach to ADHD care (continued) • Refine mapping your office flow to fully integrate the mehealth portal into your processes for: • Collecting parent and teacher Vanderbilt assessments and follow-up assessments • Initial and follow-up screening for co-morbidities • Integrate the initial conversation checklist and parent education booklet into your workflow
