  3. ADP Self Service Login Go to https://portal.adp.com and enter your username and password. Please note, the login you use for self service will be the same login you use for iPay. If you do not have a user name, you will have to register. If you have difficulty logging in or have forgotten your password, please email benefits@crcins.com.

  4. ADP Self Service Registration To register, click First Time Users Register and simple follow the prompts on each screen. The registration Pass Code is CRCInsuran-adp.

  5. Viewing 2012 Benefit Information To begin Open Enrollment, select Benefits and click Review/Change Benefits.

  6. Viewing 2012 Benefit Information You can review your 2012 elections by clicking Benefits, Compensation and Benefits.

  7. Enrolling in 2013 Benefits To enroll in 2013 benefits, click the Benefits icon, then Review/Change Benefits.

  8. Enrolling in 2013 Benefits You will then be taken to the Welcome Open Enrollment page. Please click the “show” icon to view the Open Enrollment Welcome message for important information

  9. Enrolling in 2013 Benefits Pay attention to the right side of each benefit option for helpful hints, forms and additional information pertaining to the benefit option.

  10. Enrolling in 2013 Benefits To start the Open Enrollment process, select “Walk me through this process”.

  11. Enrolling in 2013 Benefits Once you select “walk me through this process”, you will be asked if you need to add dependents. Should you need to need to add dependents, click yes.

  12. Enrolling in 2013 Benefits If you will not be adding a dependent, click start to begin with the medical benefit options.

  13. Enrolling in 2013 Benefits View your sidebar to the right for additional information before making your benefit selection. Your current medical selection may also viewed here. If you would like your benefits to remain the same, simply confirm the selection is correct.

  14. Enrolling in 2013 Benefits If you choose to modify, please do so on this screen. Click the correct medical option and coverage you would like for 2013. If adding a dependent, go to the “Adding a Dependent” section of this PowerPoint.

  15. Enrolling in 2013 Benefits The next benefit selection will be Dental. If you choose to modify, you will do so on this screen. Click the correct medical option and coverage you would like to elect for 2013. Don’t forget to view to view the sidebar for additional information.

  16. Enrolling in 2013 Benefits After making your dental selection, you may click continue to move to vision.

  17. Enrolling in 2013 Benefits Vision is your next benefit selection. For additional details pertaining to vision, view the sidebar to the right. Click the correct vision option you would like to elect for 2013.

  18. Enrolling in 2013 Benefits The next benefit option will be Healthcare FSA. You should enter your annual amount up to $2,500 as Self Service will calculate your bi-weekly amount based on 24 pay periods. If you choose not to elect this benefit option, you may click continue to move to the Dependent Care FSA benefit option.

  19. Enrolling in 2013 Benefits The next benefit option will be Dependent Care FSA. You should enter your annual amount up to $5,000 as Self Service will calculate your bi-weekly amount based on 24 pay periods. If you choose not to elect this benefit option, you may click continue to move to the AFLAC benefit option.

  20. Enrolling in 2013 Benefits The next benefit option will be AFLAC. Be sure to select the correct AFLAC plan(s) you would like to choose and the coverage.

  21. Enrolling in 2013 Benefits Please remember if you choose a AFLAC benefit, you must complete the appropriate application and arbitration agreement. All information, rates and forms are in the right side bar for your convenience.

  22. Enrolling in 2013 Benefits Applications and agreement must be submitted to the benefits department by December 7, 2012. If you choose not to elect this benefit option, you may click continue to move to the Parking and Transportation FSA benefit option.

  23. Enrolling in 2013 Benefits Here you have two benefit options, Parking and Transportation FSA. For Parking, you may enter an annual maximum up to $2,880 and for transportation, you may enter an annual maximum up to $1,500. Self Service will automatically calculate you bi-weekly premium based on your annual selection based on 24 pay periods.

  24. Enrolling in 2013 Benefits If you choose not to elect these benefit options, you may click continue to move to the Supplemental Life Insurance benefit option.

  25. Enrolling in 2013 Benefits Your next benefit option will be Supplemental Life Insurance. Lincoln has provided CRC with a “True Open Enrollment”. If you currently have this benefit, you may increase or decrease your coverage amount in increments of $10,000.

  26. Enrolling in 2013 Benefits If you elect or modify Supplemental Life Insurance, you must complete the enrollment form. Additional information for this benefit, including rates and enrollment form, can be found in the right sidebar. Enrollment Forms must be submitted to the benefits department by December 7, 2012.

  27. Enrolling in 2013 Benefits If you choose to elect or modify you Spousal Supplemental Life Insurance, you must first elect this benefit for yourself. Additional information for this benefit including rates and enrollment form can be found in the right sidebar.

  28. Enrolling in 2013 Benefits If you choose to elect or modify Supplemental Life Insurance for your child, you must first elect this benefit for yourself. Additional information for this benefit including rates and enrollment form can be found in the right sidebar.

  29. Enrolling in 2013 Benefits Your next benefit option is ARAG Legal. For additional information on this benefit, please see the right sidebar. Once you have made your selection, click continue.

  30. Enrolling in 2013 Benefits Your last benefit selection will be 401(k). Enter a dollar amount or percentage that you would like to have deducted from your bi-weekly paycheck. Please keep in mind the annual maximum for 2013 contributions is $17,500 and the catch up contribution is $5,500 for a total of $23,000.

  31. Enrolling in 2013 Benefits When making your 401(k) selection, please view the additional information provided to you in the right sidebar. Once you have made your selection, click continue to move to your 2013 Benefit Summary.

  32. Adding a Dependent After selecting “yes” in the “Walk me through this process box”, click start to add a dependent.

  33. Adding a Dependent Enter all of the dependent information requested.

  34. Adding a Dependent To add additional dependents, click “add another dependent” at the bottom of this screen.

  35. Adding a Dependent After all dependent(s) have been added, click continue.

  36. Adding a Dependent To add a dependent(s), click the New Dependent link below the correct benefit.

  37. Adding a Dependent Select your dependent tier from the drop down coverage menu.

  38. Adding a Dependent Enter all dependent information requested and click add.

  39. Adding a Dependent Any dependent(s) added to your plan will appear below your benefit selection.

  40. Opting out of a Benefit If you would like to opt out of a benefit, you should click “Decline” at the top of the screen above the benefit options.

  41. Deleting a Dependent If you choose to delete a current dependent, you may do so by simply unchecking the correct dependent(s) being removed from the benefit.

  42. Reviewing/Printing 2013 Benefit Summary After you have completed your 2013 benefit selections, you will be taken to a Benefit Summary which should reflect all benefit selections.

  43. Reviewing/Printing 2013 Benefit Summary Review your selections to ensure the benefits you intended to elect are reflected the summary. Ensure coverage tier, dependent(s) and dollar amounts are accurate.

  44. Reviewing/Printing 2013 Benefit Summary If your selections are accurate, then click Submit to Administrator for processing.

  45. Reviewing/Printing 2013 Benefit Summary To print your Benefit Summary, click the benefit icon in the top menu bar and then click Review/Change Benefits.

  46. Reviewing/Printing 2013 Benefit Summary You will be taken to the Welcome page. Select “View/Print Benefits Statement”.

  47. Reviewing/Printing 2013 Benefit Summary You will be asked to choose a benefit summary from the list. To view your 2013 benefit selections, you should choose to “View my benefits on a specific date”, enter 01/01/2013 and click go.

  48. Reviewing/Printing 2013 Benefit Summary Review your Benefit Summary and click print. Be sure to keep a printed copy as confirmation of benefits and for future reference.
