  3. Happy Anniversary! • August 2018 – Prioritization change to end Chronic Homelessness • November 2018 - "That's None of Your Business:" Gathering Sensitive Info to Get People Housed • February 2019: Landlord Engagement • May 2019: Panel discussion on community resources outside of CE

A look back – August 2018 • Our Priority List showed: • 298Chronically Homeless households • 207CH Singles • 91CH Families • OurRRH list was not prioritizing CH households • We had a PSH Families listwhere people were going nowhere • 67% of families waiting for 15%of family vacancies • 17%INCREASE in Chronically Homeless HH on list from January-August 2018

  5. Changes we made • PrioritizedChronically Homeless households for all interventions • CombinedPSH and RRH Families into one list • ExpandedCase Consultation phone calls • Beganlist clean-up

  11. Maintaining progress, increasing support • Case consultations will be the mechanism to fill project vacancies • Assessors are following-up with clients • Coordinated Entry Navigator will become a full-time position in January • Exit planning support offered at CE workshops

  12. What’s next? • Keep doing what we’re doing! • If we continue the same pace, we will be looking at ending Chronic Homelessness in 6 months! • System Performance Measures

  13. System Performance Measures • HUD’s way of “grading” the CoC • Data from HMIS is pulled from all project entry/exits • Not just HUD programs! • Shelter, RRH, PSH, and Outreach! • Measures: • Length of Time Homeless • Returns to Homelessness • Exits to Permanent Housing

  19. Summary • We’re doing good work, and we’re on the path to END Chronic Homelessness! • Study up on the 3.917 question series! • Ask for help!
