Understanding NCQA PPC-PCMH Scoring and Must-Pass Elements

This guide provides an overview of the NCQA PPC-PCMH scoring system, which is composed of nine standards totaling 100 points. It highlights the importance of ten "Must Pass" elements linked to levels 1, 2, or 3. A practice must achieve a minimum number of points and pass a set number of these essential elements to be recognized. If there's a disparity between the points scored and the Must Pass elements met, the practice will be assigned the lower level. Practices scoring 0 to 24 points or with less than five Must Pass elements will not achieve recognition.
