  2. What is an IPOC? Individualized Plan of Care • For patients whose frequent visits need multidisciplinary care planning • For families needing guidelines • To be used for future ED visits or admissions

Purpose of an IPOC • To create a consistent formatted guideline for staff in addressing patient’s complicated needs • To provide an avenue for clear communication of plan to ensure proper care of patient • To provide consistency in handling patient needs • To provide multidisciplinary expertise • To alleviate staff dissatisfaction and improve patient satisfaction

  5. What to do next? • If you have any patients that fit the IPOC criteria: Notify IPOC team members

  6. Design team members • Will review and create plan of care

  7. Delivery Team

  8. Adjunct Members Acts as advisor to design team

  9. Trigger • Entry of patient’s medical record number in ER • Carelink pages the Administrative Supervisor • “IPOC Alert” with MR last 4 digits • AS locates patient and/or confirm entry to ED • AS retrieves plan from AS “Q” drive • Delivers and reviews plan with the unit Supervisor will round and check on patient and staff regarding issues or successes of plan

  11. Review of IPOC after Discharge • Design Team will meet with Unit staff, AS and other disciplines involved • Questionnaire will be filled by staff prior to meeting with team • Team will revisit plan, make changes with staff input. • Team will save plan for next visit
