  5. Hinge @ Post 2 Oriented Toward Post 1 – Installed Backwards - Note Post Ripped off Hinge Instead of Rotating Ripped off Hinge Hinge toward Post 1

  6. Hinge @ Post 2 Oriented Toward Post 1 – Installed Backwards Note Guardrail Bolt Pealed Off

  7. Bolts For Cable Anchor Bracket Improperly Inserted thru front of Guardrail instead of from behind – therefore Bracket could not release Properly

  8. Guardrail was Improperly Attached at Post 3 and Bolt Ripped Out

  9. Guardrail was improperly attached at Post 3 the RailFolded and Pulled Bolt Out

  10. Guardrail Folded at Post 4

  11. Vehicle Tracking After ImpactFollow Oil Trail

  12. Impact head Final Location – Landed or Moved Here??
