  5. Program Director (PD) Nomination • Required items when submitting a PD nomination: • PD nomination form • Curriculum Vitae • Copy of signed Instructor / TSF card (front & back) • Copy of Core Instructor Course certificate • Training Site Faculty (TSF) nomination form

Program Administrator Nomination • Send in the PA nomination form signed by MTN appointed Program Director • PA Orientation date required • Training Site should have a PA Orientation Program in place, MTN handbook pg 40 • Report changes of TS staff (PD and/or PA) to the MTN in writing within 10 days of notice

  11. Reports • Due to the MTN by 31 Mar/30 September • Ensure all forms are filled out completely and signed • (do not leave any blank spaces) • Early turn in of packets is highly encouraged. • Packets submitted late are: • At risk for program being put on hold

  12. Annual Report • Fax Annual Report to MTN to include the following: • Annual/Semi-Annual Report Form • Instructor List • Satellite List • Agendas • Equipment list
