Indicaties • 1.      Therapieresistente positieve en negatieve symptomen bij schizofrenie • 2.      Therapieresistente schizofrenie bij kinderen en adolescenten • 3.      Onbehandelbare extrapiramidale bijwerkingen op antipsychotica bij patiënten • 4.      Psychotische aandoeningen die optreden tijdens het beloop van de ziekte van Parkinson, in gevallen waar standaardtherapie heeft gefaald. • 5.      Onbehandelbare tardieve dyskinesie en tardieve dystonie.

  4. Indicaties • 6.      Therapieresistente schizoaffectieve stoornis, bipolaire stoornis en depressieve stoornis met psychotische kenmerken. • 7.      Therapieresistente agressie bij schizofrenie of schizoaffectieve stoornis. • 8.      Therapieresistent middelenmisbruik/afhankelijkheid bij schizofrenie of schizoaffectieve stoornis. • 9.      Recidiverend suïcidaal gedrag bij therapieresistente en niet-resistente schizofrenie en schizoaffectieve stoornis. • 10.  In uitzonderlijke gevallen: therapieresistente agressie en/of automutilatie bij borderline persoonlijkheidsstoornis, autisme of ernstige zwakzinnigheid.

  9. Patiënten • Chronische schizofrenie/schizoaffective stoornis (DSM III-R). • Patiënten zijn niet verbeterd na minimaal 2 soorten antipsychotica. • Geen eerder clozapine gebruik.

  10. Patiënten • N=50 11 vrouwen en 39 mannen • Leeftijd gem 38 jaar (range from 21 to 56) • Global Assessment of Functioning bij start studie (15-40) (gem. 29). • 2 - 44 opnamens (gem.10) • 2 - 33 jaren na eerste opname (gem. 15)

  11. Interventie • Gerandomiseerd 3 groepen serum concentraties clozapine Range N Gem SD I 50-150 16 94 (13.5) II 200-300 22 248 (15.0) III 350-350 12 391 (26.8)

  12. Outcome • 21-kanaals EEG • 0-4 schaal van afwijkingen (anchored 5-level ordinal scales) • EEG gemaakt na 10 weken clozapine • 0  geen afwijkingen • 4  convulsie

  13. resultaten • 24/45 patiënten (53%)  enige vorm van EEG abnormaliteit. • 15/45 patiënten (33%)  nieuwe EEG afwijkingen significant serum concentratieafhankelijk verdeeld • 1/48 patiënten  convulsies (2%) • I 20% • II 21% • III 73% ** (F = 4.0; df = 2,42; p = .025)

  14. Conclusie • Serum concentratie clozapine is geassocieerd met EEG afwijkingen en daarmee ook met convulsies.

  15. Incidentie convulsies • Incidentie clozapine geïnduceerde insulten is 3% en lijkt dosis gerelateerd. (1,2) • 1418 USA patiënten incidentie insulten 2.8% cummulatief na 10 jr 10%. (2) < 300 mg/d (1%-2%) (3) 300-600 mg/d (3%-4%) 600-900 mg/d (4%-6%)

  16. Richtlijn convulsies bij clozapine gebruik • Eerste insult: -Halvering van de dosis clozapine en spiegelbepaling. -Tijdens instellen het minderen van benzodiazepines vermijden. Tweede insult: -valproaat toevoegen, -evt. consult neuroloog
