  6. Southampton SEND PathfinderOne and Only Pack • As far as possible, there should be a ‘tell us once’ approach to sharing information, so families and young people don’t have to repeat the same information to different agencies, or different practitioners/services within each agency.

Southampton SEND PathfinderKey Working: Assessment Co-ordinator • Supported through the assessment process by a single point of contact • Children, families and young people are at the centre of the assessment and planning process and should be partners • The assessment process should be co-ordinated, person centred, timely and without duplication.

Southampton SEND PathfinderKey Working: Lead Professional • Builds on previous assessments and continues to identify and meet the needs of children, young people and families as they develop • Annual person-centred reviews • Practitioners should be engaged and committed to the co-ordinated assessment and planning process and where necessary, trained in person centred approaches.

Southampton SEND PathfinderEducation, Health and Care Plans • EHC plans should be person centred, evidence-based and focussed on outcomes • EHC plans should support preparation for key transition points. Plans must be “forward looking” • EHC plans should explore how informal (family and community) support as well as formal support from statutory agencies can be used to achieve agreed outcomes

Southampton SEND PathfinderVisions becoming reality • This is Tom, he is 2 ½ years old • He was born prematurely and he has • Chromosome deletion resulting • in: • Hearing difficulties • Vision difficulties • Epilepsy • Unsafe swallow • Profound & multiple needs • After he was born he stayed in • hospital for 6 months before • he was able to come home. • This is Tom’s Family • They were very shocked when Tom was born early. • Their primary concerns are: • Maintaining his gastrostomy tube feed (safety) • Accessing continence resources • Having ideas for play at home • Respite to get a good night’s sleep • Time for siblings • Thinking about where Tom might go to pre-school, and then school • Conflicting appointments • Having to tell people their story over, and over, and over again Epilepsy Nurse New-Born Hearing Check Initial Assessment Community Childrens Nurse Eye Unit Health Visitor Portage Assessments Ophthalmology consultant Paediatric Assessments Short Breaks Assessment Neurology consultant Child Development Unit Continence Assessment Therapy Assessments Respite services Continence Nurse Portage Social Care Health Surveillance Checks Core Assessment CCN Assessments

Southampton SEND PathfinderTom and his EHCP Application made by Community Paediatrician for anEHC Assessment Timely reviews ensure the EHC plan continues to meet Tom’s needs Key transition points anticipated and planned for (for example; when Tom starts pre-school) Assessment Co-ordinator allocated to be the single point throughout the assessment Lead Professional identified to be the single point of contact for Tom and his family for the duration of his plan Tell us once approach “One and Only” suite of documents owned by Tom and his family Bespoke, quality assessment programme planned and delivered in partnership with the family More choice and control for Tom and his family taking up direct payment for their short breaks Professionals from education, health and care planning together and in partnership with families to meet outcomes Resources specified and allocated to support Tom to meet his outcomes in line with the Local Offer
