6. Building Blocks for SIM • The Colorado Health Care Innovation Plan Combines Core Elements to Produce: • Payment reform coupled with care redesign. • Plan to improve population health and patient care experience and control costs. • Statewide impact.

Our Vision & Goals • To create a coordinated, accountable system of care that gives Coloradans access to integrated primary care and behavioral health in whichever setting counts as the patient's medical home. • To leverage the power of our public health system to achieve broader population health goals and support delivery of care. • To use outcomes-based payments to enable transformation. • To engage individuals in their care.

Our Approach • Integrated Care: “The care that results from a practice team of primary care and behavioral health clinicians, working together with patients and families, using a systematic and cost-effective approach to provide patient-centered care for a defined population. This care may address mental health, substance abuse conditions, health behaviors (including their contribution to chronic medical illnesses), life stressors and crises, stress-related physical symptoms, ineffective patterns of health care utilization.” (Peek, 2013)

Project Update • Where We Have Been • Colorado received $2 million “pre-testing” award from the Center for Medicare & Medicaid Services for the State Innovation Model Initiative (SIM). • A number of stakeholder workgroups were involved in the development of the original proposal. • In December 2013, a Health Care Innovation plan was submitted to the federal government as part of “pre-testing” award. • Since then, we have been waiting for more information on the next round of funding. And now we have it…
