  2. Going Places Pick up form from Mrs. McInnis & get her signature. Fill out requisition if $$ are needed Give form and requisition to Mr. Smith. Outside Points Fill out the outside points form Attach certificate or documentation Turn in to Mrs. McInnis Forms & Points
