The problem of pressure ulcers • The development of HAPU places the patient at risk for other adverse events and increases resource consumption and healthcare costs. • Interventions to reduce pressure, friction/shear and identify risk factors such as immobility, incontinence, impaired nutrition will decrease pressure ulcer occurrence and he worsening of existing pressure ulcers. • Pressure ulcer prevention requires multidisciplinary effort
Prevalence Study • Point prevalence • Includes those admitted to the healthcare facility with a pressure ulcer and those who developed one between admission and the time of the study. # of pt’s with a PU at a particular point in time divided by the total # of pt’s in the population studied at a particular point in time multiplied by 100
Purpose • To determine the rate of pressure ulcer occurrence • To determine the proportion of patients who received skin and pressure ulcer risk assessments • To determine the proportion of at risk patients who received pressure ulcer prevention • To determine the type of prevention interventions implemented for at risk patients.
Goal: ACCURATE and RELIABLE data collection • Each facility must identify persons to collect data for a PU Survey • Head Team Leader • Team Members • Support Staff
The Study • Quarterly PU surveys are recommended • Must be conducted on a designated day on all units • List of all patients assigned to the unit • Number of patients excluded from study
Process For each patient assessed • Skin assessment documented within 24 hours of admission? • Braden recorded within 24 hours of admission • Admit Braden score • Date and time of last Braden score prior to survey • Based on the last Braden, was patient at risk? • Pressure prevention protocol in place for at risk patients?
Types of prevention interventions used within the last 24 hours • Skin assessment within last 24 hours? • Pressure redistribution surface? • Repositioning? • Nutritional support? • Moisture management?
Pressure Ulcers • # of pre-existing Pressure Ulcers- should be staged and reported • # of Hospital acquired Pressure Ulcers- staged and reported • Unit of Origin of Hospital Acquired PU
After Data Collection • EXTERNAL; submit data to external databases • Internal: calculate HAPU rates • # of patients with HAPU divided by the # of patients in the survey multiplied by 100
