  2. 2 casus • Casus 1: • 46 jarige vrouw • Acute hoofdpijn, misselijkheid en braken • CT-hersenen: SAB, CT-A: niet coilbaar aneurysma a. communicans anterior • Klaagt over visus problemen

  3. 2 casus • Casus 2: • 61 jarige vrouw • Acute hoofdpijn, misselijkheid en braken • CT-hersenen: SAB, CT-A: niet coilbaar aneurysma a. carotis interna • Klaagt over visus problemen

  4. Syndroom van Terson?????

  5. Intraoculaire bloedingen bij een SAB • Retinaal • Pre-retinaal • Subhyaloidaal • Glasvocht (Terson)

  6. Syndroom van Terson • Glasvocht bloedingen bij intracerebrale bloedingen (subarachnoidaal, subduraal) • Voor het eerst beschreven in 1881 • Discussie over wat syndroom inhoud • Alle intraoculaire bloedingen of alleen glasvocht bloedingen

  7. Incidentie • Oculaire bloedingen 10-40% • Glasvocht bloedingen 8-14%

  8. Mechanisme • Controversieel, 2 gezichtspunten: • Direct vanuit subarachnoidale ruimte door lamina cribrosa • Ten gevolge van verhoogde ICP verminderde veneuze terugvloed  veneuze stase  veneuze bloedingen

  9. Relatie met de ernst van de SAB, significant hogere Hunt en Hess score bij glasvochtbloeding. • Mortaliteit 43% vs 9%

  10. Beloop • Studie 70 patienten • Patienten met glasvochtbloeding slechtste visus. • 76% herstelde naar een visus van 0.6 of meer.

  11. Behandeling • Vitrectomie • Met name bij bilaterale glasvochtbloeding (revalidatie) • Bij complicaties (retina loslating, ontwikkelen epiretinaal membraan) • Kinderen (amblyopie)

Samenvattend • Intraoculaire bloeding vaak voorkomend bij SAB • Glasvochtbloeding meest beperkend voor de visus • Glasvochtbloeding geassocieerd met slechtere Hunt en Hess en slechtere outcome • Spontaan herstel kan worden afgewacht, operatief ingrijpen bij: • Bilateraal ernstig visus verlies • Late complicaties • Kinderen
