  8. Fill in today’s date • Fill in your Student ID#

  9. Fill in your First Name As an example, this student’s name is “John Smith” • Fill in your Family Name

  10. Please locate your Visa Type (this will be located on the front of your Visa) Front of Visa 

  11. Circle the correct choice on your check-in form for your Visa Type

  12. Port of Entry – Which U.S. Airport did you land at when you arrived? • Fill in the 3 letter code for the port of entry of your arrival on the check-in form  In this example it isSFO

  13. Find in the date that you entered the U.S. • Fill in the date on your check-in form  In this example it isSEP 17 2013

  14. Find your I-20 form and locate the expiration date • Fill in the date on your check-in form (this should be in month/day/year format)

  15. Fill in the SEVIS number on your check-in form In this example it is N0000110429

  16. Mark an “X” in the box for the type of housing you are staying in while attending AEI • Fill out your address information House number (in this example it is 123) Street Name (in this example it is Jefferson Street) Apartment number (in this example it is 6) City (in this example it is Eugene) Zip Code (in this example it is 97405)

  17. Fill in your U.S. phone number In this example it is 541-123-4567 • Fill in your personal e-mail address In this example it is myemail@gmail.com

  18. Your check-in form is now complete!
