Box 126 Portage, IN 46368 Portage Athletic Booster Club Membership Benefits 2013-2014 Membership Form • Membership is for one year beginning July 1, 2013 through June 30, 2014 • Brave Level - $25.00 • Voting rights on booster proposals Warrior Level - $125.00 • All Brave Level benefits • One athletic pass valid at all PHS home games in 2013-2014 Chief Level - $200.00 • All Brave Level benefits • Two athletic passes valid at all PHS home games in 2013-2014 PHS Athletics The Portage Athletic Booster Club’s fundraising projects include membership fees, apparel sales, and other advertising opportunities. Our funds and volunteer work have helped provide assistance to all twenty athletic programs as well as facility improvements. For example, the booster club has provided funds to purchase uniforms, weight room equipment, and our current bigger, faster, stronger training program. We cannot succeed without your help!
