Curriculum Reform: “Preclerkship” Major Themes HMS Academy New paradigm of learning; emphasize personal accountability, life-long learning, thinking & analysis Structure: principles of biomedical, population and social sciences pre-clerkship with advanced sciences post clerkship Early clinical experiences integrated with core sciences Periodic, cumulative, multifaceted assessment

Organization of Curriculum Jul Aug Sep Oct Nov Dec Jan Feb Mar Apr May June Year 1 Foundations Organ System Blocks  Year 2  Organs Principal Clinical Experience  Year 3 Advanced Selectives  PCE USMLE Scholarly Project Year 4 Subinternships and Advanced Electives • Advanced selectives could include advanced pathophysiology courses and integrated subjects such as genetics, cancer, nutrition, human development/aging, population health, and social medicine • Highlights • Moves principal clinical year earlier into Year 2, allowing for early clerkship exposure • Acknowledges core basic science needed prior to clinical year but that richness of more advanced science is best suited to students post-wards • More individualized learning; dedicated scholarship block; advanced topics based on interest • Pushes USMLE to Year 3, allowing for focused attention on core basic science in Year 1 HMS Academy

Design Phases (draft) HMS Academy Phase 1: Broad overview of year 1 courses (what goes into “Foundations”?); key questions to focus content (Oct 13-March 14) Phase 2: Detailed design of courses; writing the curriculum (syllabus, cases, student assignments); design of assessment and evaluation exercises/mentoring (April-Dec 2014) Phase 3: Design of advanced basic science Simultaneous faculty development and program assessment

Key Questions (how? why?) for Each Course HMS Academy • Example: Respiratory • Anatomy: How does anatomy of chest wall, lung pleural contribute to creation of intra-thoracic pressure? • Physiology: How does physiology of the lung/chest wall/airways interact to determine airway resistance? • Pathophysiology: How do pathologic changes in the lung and chest wall contribute to decreased compliance of the respiratory system? • Pharmacology: How does pharmacology of aminophylline determine its dosing, toxicity, effects?
