  12. GISAI Data • There are approximately 12500 accident cases with an accident/ penalty[1] report reported between January 1998 and February 2004. • This is an underestimate of the total number of work related accidents as many go unreported [1] Sometimes referred to as prosecution-reports

Accident frequency per year • In the 6 year period analysed there were per year on average: • 425 contact with moving part of a fixed machine accidents • 220 hit by falling objects of which 169 not crane related, 51 crane related • 195 contact with flying/ejected objects • 178 fall from height roof/floor/platform • 173 ladder fall accidents of which 122 mobile/placement ladders • 89 struck by moving vehicle • 86 fall from scaffold accidents • 63 Loss of containment accidents of which 40 from normally closed containments • 31 contact with electricity

  14. 1.1.1. Ladder accidents(reported and investigated) • Every year there are on average 4 deaths from falls from a ladder, 22 permanent injuries and 88 recoverable serious injuries Deaths 1 Permanent injuries 6 Recoverable injuries 22

  15. 1.1.1.1 Mobile ladder accidents The three mutually exclusive barrier failure types that directly led to falls:- • 67% LADDER STABILITY fails: • Mostly due to setting it up incorrectly • The rest due to • an unstable surface • being hit by something • poor position of the person on the ladder • 29% USER ABILITY fails • mostly due to loss of control over body movement such as due to the force of using a tool or a misstep • 4% LADDER STRENGTH fails • Due to substandard condition or type of ladder

  16. 1.1.1 Ladders - MOTIVATION • In 50% of ladder accidents, lack of motivation for safety played a role, soLACK OF: • Alertness, care & attention, • Concern for safety of self & others, • Risk avoidance, • Willingness to learn & improve

  17. 1.1.1. Ladders - CONFLICTS • Conflicts with ‘profit’ goals played a role in 20% of ladder accidents ….in around 200 accidents, barriers were compromised in association with safety-profit conflicts e.g. 83 cases of conflicts with correct placement of the ladder

  18. 1.1.1 Ladders- MISMATCH • In almost 50% of the ladder accidents, mismatch between the activity being undertaken and the tool or method used played a part. e.g. the ladder was too short Arbobesluit art. 7.3 lid1&3 en Art.3.16 lid1.
