Main Database • Ingress to CT4 Database Migration QC’d • No issues with found • Database revisions ongoing • Database being revised for the following forms: • Birth History • Family History • Genetic Physical History • Cardiology • Extra-cardiac Findings • Cytogenetics • Revision completion expected 3/22

Main Database (cont’d) • Data Entry • Data Entry Guidelines being revised • Data Entry expected to re-start 3/26 • Revisions to previously entered data • Many of the revisions being programmed significantly impact previously entered data • Ex. Check one questions changing to check all • Fields impacted by this are being reviewed using listings from current structure • Data being updated into the new structure as appropriate

Project 3 Database • Forms are being refined in parallel with database creation • Three (3) forms are considered complete and are being programmed: • Exercise Stress Test • Programming completion expected 3/30 • Electrophysiology • Programming completion expected 4/30 • Study Information • Programming completion TBD

  5. Project 3 Database (cont’d) • Other forms are being revised prior to starting programming: • Cardiac Catheterization (Pre & Post) • MRI • Post-operative Echocardiogram • Medical Assessment & Hospitalization • Perioperative (2 groups of forms) • Questionnaires (CHQ-CF87, PCQLI, etc.)
