  2. Renewal Increase 2013: +5.9% Actual bill rate affected by - Age Rating, - Tier Changes, and - Line Item Billing

  3. Rate Level Adjustments • 15% of Groups One level UP • 15% of Groups One level DOWN • 70% remain on the SAME rate level (one level = 7.5%)

  4. Renewal History… 2004 – 0.0% 2005 - 9.9% 2006 - 6.9% 2007 - 11% 2008 - 4.2% 2009 - 8.9% 2010 – 11.5% 2011 – 13.9% 2012 – 1.1% 2013 – 5.9% Average – 7.33%

  5. Group Premium Invoice Effective 1/1/2013, continuing to separate line items on bill showing: • $.90 PEPM for UNUM (Life, AD&D, EAP, Will Preparation) • $1.46 PEPM for VSP Vision Exam Benefit • $3.14 PEPM for BCBS billing fee, Wellness, Marketing, Administration • TOTAL: $5.50 PEPM

  6. 2013 Benefit Changes PPO Plans – Out-of-network Co-insurance CMM 50/50 Plan ($1,000 Ded., $4,000 OOP) – discontinued Health First 50/50 ($1,000 Ded.) & 80/20 ($1,500 Ded.) – New Drug Card option: Full or Pass thru

Women’s Preventive Services • Well woman visit • Screening for gestational diabetes • Human papillomavirus (HPV) testing • Counseling for sexually transmitted infections • Screening and counseling for HIV • Contraceptive methods and counseling • Breastfeeding support, supplies, and counseling • Screening and counseling for interpersonal and domestic violence

Additional Benefit Reminders • UNUM $5,000 base life insurance and AD&D for all covered employees (Buy-up available) • UNUM Employee Assistance Program for all members (including legal) • $10 VSP vision exam for all covered members (Buy-up available) • $100 Preventive Dental paid under medical for all covered members
